Objective: Endometrial carcinoma is one of the most common malignant tumors of female reproductive tract, in which endometrioid adenocarcinoma is the most common. Recent studies show that, the formation of new blood vessels play an important role in the occurrence and development of this disease. Vascular endothelial growth factor(VEGF) is currently recognized as one of the vascular growth factors that have a wide and strong role. Ephrin B2 is an important angiogenesis factor found in recent years,and involved in important physiological functions, like cell growth, cell differentiation, embryonic development and signal transduction and so on, play an apparent role in regulating the process of many tumor angiogenesis. The studies show that VEGF can enhance the effect of ephrin B2, and blocking ephrin B2 could inhibit the endothelial cell sprouting process VEGF induced. This experiment mainly is to detect the different expression of angiogenesis factors VEGF protein and ephrin B2 protein in normal endometrium(NE) and endometrioid adenocarcinoma(EA) tissues, futher to explore the correlation of their expression and investigate the relations of both proteins with clinical pathological index, such as tumor differentiation, infiltration, lymph node metastasis and pathological staging.Method:1 Collected 32 cases of endometrioid adenocarcinoma tissue specimens as researching group and 10 cases of normal endometrium tissue specimens as contrast group,which were all obtained by gynecological surgical resection in the Second Hosipital of Hebei Medical University. According to FIGO2009 operation pathologic staging of endometrial carcinoma:there were 14 cases in stage Ⅰ, 18 cases ≥stage Ⅱ;to the situation of lymph node metastasis:6 cases with lymph node metastasis, 26 cases without lymph node metastasis;to the invasion depth of muscular layer:18 cases ≤1/2 muscular layer,14 cases >1/2 muscular layer;to the degree of pathological differentiation:10 cases in high differentiation(G1), 12 cases in middle differentiation(G2) and 10 cases in low differentiation(G3).2 Used immunohistochemical staining method to detect the expression of VEGF and ephrin B2 protein. Randomly selected 5 HPF(×400) in the area of positive expression,analyzed images by Image-pro plus(version 6.0)software,and measured the value of IOD(Integrated Optical Density)of the positive area in every vision,then took the average value of IOD。The relations of these two proteins and the relationship between their expressions and clinical pathological index were analyzed.3 SPSS19.0 was applied for statistical data analysis. Used different statistical methods according to the data: two sample mean comparison by T test; multiple samples mean comparison by analysis of variance; correlation by Pearson correlation analysis. P<0.05 was statistically meaningful.Results:1 The IOD value of VEGF protein in 10 cases of NE and 32 cases of EA respectively is 1.43E5±0.26E5、3.95E5±1.05E5. There is significant difference in statistical analysis(P=0.00<0.05).2 The IOD value of VEGF protein in different operation pathologic staging of endometrial carcinoma,14 cases in stage Ⅰ,18 cases ≥stage Ⅱ,respectively is 3.39E5±0.89E5、4.38E5±0.98E5,there is significant difference in statistical analysis(P=0.006<0.05).The IOD value of VEGF protein in 6 cases with lymph node metastasis, 26 cases without lymph node metastasis,respectively is 5.05E5±1.05E5、3.69E5±0.89E5,there is significant difference in statistical analysis(P=0.003<0.05). The IOD value of VEGF protein in 18 cases ≤1/2 muscular layer,14 cases>1/2 muscular layer,respectively is 3.52E5±0.81E5、4.50E5±1.09E5, there is significant difference in statistical analysis(P=0.007<0.05). The IOD value of VEGF protein in the different degree of pathological differentiation,10 cases in G1, 12 cases in G2 and 10 Case in low G3,respectively is 3.56E5±1.07E5、3.96E5±0.96E5、3.96E5 protein is. It is concluded that ephrin B2 protein is related to the occurrence and local invasiveness of EA.3 The expression level of VEGF and ephrin B2 protein is positively related. It suggests that VEGF, ephrin B2 protein participate in the angiogenic process of EA. Put up a new way for the treatment of EA. ±1.05E5,there is no significant difference in statistical analysis(P=0.274 > 0.05).3 The IOD value of ephrin B2 protein in 10 cases of NE and 32 cases of EA respectively is 0.64E4±0.24E4, 1.64E4±0.82E4. There is significant difference in statistical analysis(P=0.00<0.05).4 The IOD value of ephrin B2 protein in different operation pathologic staging of endometrial carcinoma,14 cases in stage Ⅰ,18 cases ≥stage Ⅱ,respectively is 1.65E4±0.72E4 、 1.64E4±0.91E4, there is no significant difference in statistical analysis(P=0.979>0.05).The IOD value of VEGF protein in 6 cases with lymph node metastasis, 26 cases without lymph node metastasis,respectively is 2.01E4±0.91E4 、1.56E4±0.79E4,there is no significant difference in statistical analysis(P=0.226>0.05). The IOD value of VEGF protein in 18 cases ≤1/2 muscular layer,14 cases>1/2 muscular layer,respectively is 1.20E4±0.64E4、2.20E4±0.68E4, there is significant difference in statistical analysis(P=0.000<0.05). The IOD value of VEGF protein in the different degree of pathological differentiation,10 cases in G1, 12 cases in G2 and 10 cases in low G3, respectively is 1.57E4±0.97E4、1.34E4±0.74E4、2.08E4±0.61E4, there is no significant difference in statistical analysis(P=0.101 > 0.05).5 By the analysis of linear correlation we find:the expression level of VEGF and ephrin B2 protein is positively related.(r=0.59, P=0.00 < 0.05).Conclusion:1 In EA VEGF protein expression level is much higher than that of in NE. The deeper the myometrial invasion is,with the lymph node metastasis, the higher the surgical pathologic stage is, and the higher expression of VEGF protein is. It is suggested that VEGF protein is related to the occurrence and development of EA, and VEGF protein expression level reflects the malignancy degree of EA. Furtherly it is associated with malignant biological behavior of EA and has certain prognostic value.2 In EA ephrin B2 protein expression level is much higher than that of in NE.
