The Clinical Research Of Fumigating And Washing Of Shujinhuoxue Combine With Shock Wave In Treating The Metatarsal Stress Fractures

Purpose:This study focuses on whether the therapeutic effect of fumigating and washing of shujinhuoxue combined with shock wave therapy on treating metatarsal stress fracture is better than that of pure shock wave treatment by comparing the two treatment regimens.Methods:All the 46 patients were diagnosed with metatarsal stress fracture with X-ray, CT or ECT examination in Fuzhou General Hospital from February 2013 to October 2014. According to the order of visiting, they were randomly allocated into group of combined and group of shock wave with the random number table method,thus 23 cases in each group. The patients in group of combined received medium doses of shock wave for once every day with 2000 pulse every time,2 days in a row. They were treated with fumigating and washing of shujinhuoxue after the shock wave therapy, for 30 min once a day, and every 4 weeks as a course of treatment, and patients totally received one course.To avoid weight bearing in the following 4 weeks after treatment,the patients could walk on crutches in their daily life; and they could walk as usual after 4 weeks, but still avoided training using lower limbs until the fracture healing. While the patients in group of shock wave received the same doses of shock wave for once every day with 2000 pulse every time,2 days in a row and other treatment regimens were the same as group of combined. Patients were required to receive the X-ray and CT examination every 4 weeks until the osseous healing of fracture,and the grade of callus, DOMS after 4 weeks,DOMS=0 level time and THT (treatment-healing-time) value were recorded.Results:1)4 weeks after treatment:through comparison of callus grade and DOMS Within the group, two treatment regimens were effect on the promoting fracture healing and relieving of muscle fatigue (p<0.05); through comparison of callus grade and DOMS between groups, group of combine was effect on the relieving of muscle fatigue and promoting fracture healing (p<0.05).2) After the treatment, by comparison between groups with measuring the time of DOMS=0 level andTHT, fumigating and washing of shujinhuoxue combined with shock wave in the treatment of metatarsal stress fracture, the improvement of front feet fatigue is better than that of pure shock wave therapy (p<0.05). fumigating and washing of shujinhuoxue combined with shock wave in the treatment of metatarsal stress fracture, the improvement of promoting fracture healing is better than that of pure shock wave therapy (p< 0.05); 3) Follow-up time is 5～12 months, an average of 7 months.46 cases received follow-up,46 cases have achieved in healing.4) Severe complications, such as blood vessel and nerve damage,did not happen during the Shock wave treatment of stress fracture.Conclusion:Fumigating and washing of shujinhuoxue combined with shock wave has the advantages in relieving of muscle fatigue and promoting fracture healing in the treatment of metatarsal stress fracture.
