| Purpose:To observe the efficacy and safety of Hashimoto’s thyroiditis with hypothyroidism by the treatment of warming the kidney,strengthening the spleen and resolving phlegm. It also explores the role of tumor necrosis factor-alpha (TNF-alpha) in Hashimoto’s thyroiditis and the effects of drugs on indicators, whicl serves to provide a theoretical basis for clinical Chinese medicine treatment of Hashimoto’s thyroiditis.Methods:the randomly divided60patients into two groups, who are diagnosed with Hashimoto’s thyroiditis with hypothyroidism, as well as the TCM Prosperity of differentiation with kidney Yang, spleen deficiency and phlegm stagnation syndrome diagnosis standard of mutual in line with the patients.Based on conventional western medicine therapy, the30patients’ medicine of treatment group is plus with traditional Chinese medicine, one dose daily, successive for12weeks. Two groups is respectively tested the clinical symptoms, signs of differnces before and after the treatment, observation of thyroid hormone before and after treatment of thyroid hormone and related antibody levels, interleukin6(IL-6), tumor necrosis factor alpha (TNF-alpha),etc.,as well as the adverse reaction and compliance of the patients,etc.Results:The gender,age and course of disease of the patients in two groups had no difference in pretreatment(P>0.05).l.The improving function of symptoms and signs:after treatment, symptoms of two groups patients in treatment were obviously improved, with the total effective rate was93.33%;The control group’s total effectiveness is56.70%. Many TCM symptoms in the treatment group much more than those in the control group (P<0.01).2.The thyroid hormone levels:compared both before and after treatment, the treatment group and control group of FT3,FT4and TSH were significant difference (PO.01or P<0.05);Comparison between groups, the treatment group had elevated levels of serum FT3,FT4is significantly higher than the control group (P<0.05),serum TSH is better than that of control group (PO.05), All have statistical significance.3. Compared with antibody levels related to thyroid, the treatment group,control group,TGAb and TPOAb were significantly lower.The difference was statistically significant (P <0.01);Comparison between groups after the treatment, the treatment group’s TGAb and TPOAb than the control group there were significantly (PO.01) different from the control group.4. Compared with before, two group of patients have different degree of decline in the TNF alpha, and the difference was statistically significant (P<0.01);The comparison between groups, the treatment group TNF alpha dropped significantly more than that of the control group (P<0.05). IL-6after treatment in both groups all have different increases compared with before treatment (P<0.01);Comparison between tne two groups, the treatment group’s IL-6rise significantly higher than that of the control group (P<0.05). The study also found that there is significantly positive correlation in the sick group’s serum TNF alpha,TGAb and TPOAb.(r=0.88, P<0.01; r=0.81, P<0.0l).lL-6,TGAb and TPOAb had no obvious correlation (r=-0.71, P=0.59; r=-0.93P=0.48).5. Security:in this study, both of the treatment group and the control group all have good security, no obvious adverse drug reaction after taking this medicine.Conclusion:Hashimoto’s thyroiditis with hypothyroidism by the treatment of warming the kidney,strerigthening the spleen and resolving phlegm, can significantly improve the clinical symptoms and thyroid function, reduce the thyroid antibody titer, adjust the pathological state of immune disorders, Curative effects is reliable, safe. |
