Current Situation And Influence Factors Of Community Doctors’ Serviceability And Service Motivation During Gate-keeper System Implemented In Chongqing

0bjective:To investigate the situation of gate-keeper system and service abilityand motivation of community doctors in Chongqing, and analyze theinfluence of service ability and motivation of community doctors ongate-keeper. In order to provide reference of promoting the rational use ofgrassroots health resources in our country and setting up the gate-keepersystem, to propose recommendation for improving community doctors’service ability and motivation, referred to domestic and foreign experience,and combined with the fact of Chongqing.Methods:Material sources: We collected relevant articles from CNKI database,national or Chongqing’s policy documents on the system, and interviewrecords of experts in this field, people in charge of community healthinstitutions and leaderships of health department. Data were collected fromChina and Chongqing Health Statistics Yearbook, questionnaires and statistic analysis.Research methods: Literature review, questionnaires survey, in-depthinterview，expert consultation and statistical analysis (including q-squaretest and logistic regression) were used in this research.Research results:1the practical status of domestic and foreign gate-keeper system: Thispaper has analyzed the successful experience and the development patternof other country, as well as the exploration of our country.2the theoretical basis of service ability and motivation: To find thetheories about service ability and service motivation from management,psychology and histology, etc.3the current situation of gate-keeper system in Chongqing: This paperhas investigated the current situation and problems in implementation ofthe gate-keeper system, and summarized the influence indicators of thesetwo systems by statistical analysis.4the current situation of community doctors’ service ability andmotivation in Chongqing: This paper has analyzed the investigation resultsof community doctors’service ability and motivation in Chongqing, and itsinfluence in gate-keeper system.Conclusions and recommendations:The important factor is that community doctors’ service ability isunsatisfying and they are lack of service motivation to affect the implementation of gate-keeper system. We need to optimize the trainingsystem, make the title promotion system be beneficial to communitydoctors, put forward fair and reasonable compensation system and theappropriate incentive mechanism, build a kind of organization environmentand social environment to be good for community doctors’ growth andactivating their enthusiasm, and make norms to restrain communitydoctors’service behavior for ensuring positive incentives.
