The Effect Of Bascia Medicial Health Care Institution Service Capacity On Gate-Keeper System Under The Background Of Urban And Rural Overall Development

Striving to develop rural medical and health service system, and perfecting the new urban medical and health service system which is based on community health services are the important contents of deepening the reform of our medical and health care system. Improving basic health service system, perfecting the construction standard of rural hospitals and community health service center, and establishing and perfecting the first contact care in community, classification of medical treatment and two-way referral is the short-term goals of the new medical reform.With the rural three level health service network constantly improving, the first contact care and two-way referral system play a more and more imporatnt role in promoting health resources to flow rationally among rural three-level network. Thus, to establish and perfect the " two-way referral system" has a very important significance in the health and sustainable development of the basic medical and health institutions. However, in the actual operation there is a wide gap between the " two-way referral system" and national policy requirement and the actual demand of residents. In general, we hasn't formed the reasonale pattern of"visit community health service facility with small diseases, visit big hospital with serious siseases, and return to community health service to get rehanilitation services."And the impede operation of " two-way referral system " has become the bottleneck restricting the healthy development of our community health service. However, the main reason which affect the impede operation of " two-way referral system " is the basic-level medical health agency service capacity is insufficient to satisfy the health service needs of the masses.Health service is a kind of special product, whose quality is the key point affects the flow of patients. And all health services starts from patients' first contact care. if they do not get the satisfied service, the patient will choose "substitute" or comprehensive hospital for medical treatment. So service capability directly relates to whether its residents is willing to use the "products" provided, also relates to the establishment of first contact care system and two-way referral system, and affects functions of basic medical and health institutions service.This study reviewed the community health services abroad, and the development progress and the characteristics of our health and medical institutions. On this base, it chooses chongqing city as empirical site. Through the investigation result of residents, doctors, basic medical and health institutions, and combining in-depth interviews of experts, it analyzes the present service capacity and existing question of basic-level medical health agency, and the influence of two-way referral system. Finally some measures are put forword to strengthen the basic medical and health services ability.
