Application Of Protection Motivation Intervention Model For Patients With Senile Osteoporotic Vertebral Fracture

ObjectivesBased on the theory of protection motivation, to explore the effects of protectionmotivation intervention on fall risk, knowledge and self-efficacy about osteoporosisand health-related behaviors of patients with senile osteoporotic vertebral fracture,and to provide basic reference for carrying on purposeful nursing intervention byclinical nurses.MethodsFrom September2012to August2013, eighty patients with senile osteoporoticvertebral fracture, who were from a third A grade hospital in Hengyang city of Hunanprovince, were randomly divided into the control group and the intervention group bythe charged time, with40cases in each group. The patients in the control group weretreated with routine care, and the patients in the intervention group were treated withroutine care and protection motivation intervention. Before intervention, in the thirdand sixth month after intervention, the patients’ fall risk, knowledge and self-efficacyabout osteoporosis were assessed. In the sixth month after intervention, the patients’health-related behaviors were assessed. All data were analyzed and processed bystatistical software SPSSl3.0, and the statistics methods included Descriptivestatistical analysis, Independent-samples t test, Chi-square test, Profile analysis,ANOVA of Repeated Measure and so on.Results1. There were75patients with senile osteoporotic vertebral fracture who finished theresearch, there were37cases in the control group and38cases in the interventiongroup. Before intervention, in the control group and the intervention group, the scores of patients’ fall risk were respectively (40.30±4.66) and (40.39±4.07); which ofknowledge about osteoporosis were respectively (10.41±3.14) and (10.11±3.53);which of self-efficacy about osteoporosis were respectively (54.72±8.23) and (53.84±7.89). There were no statistical difference for their scores between the two groups(P>0.05).2. In the third and sixth month after intervention, the scores of patients’ fall risk wererespectively (36.03±3.32) and (33.71±2.86) in the intervention group, they weresignificantly lower than that before intervention (P<0.05). In the sixth month afterintervention, the scores of patients’ fall risk were lower than that in the third monthafter intervention. The scores of patients’ fall risk were respectively (39.49±4.04) and(39.57±4.07) in the control group, there was no significant difference between beforeand after intervention (P>0.05). At the same time after intervention, the scores ofpatients’ fall risk in the intervention group were lower than that in the control group,there were statistical difference (P<0.05).3. In the third month after intervention, the scores of patients’ knowledge andself-efficacy about osteoporosis were respectively (16.92±3.05) and (68.16±7.70) inthe intervention group; in the sixth month after intervention, the scores of patients’knowledge and self-efficacy about osteoporosis were respectively (19.13±2.18) and(74.24±5.35), they were significantly higher than that before intervention (P<0.05). Atthe same time after intervention, the scores of patients’ knowledge and self-efficacyabout osteoporosis in the intervention group were higher than those in the controlgroup, there were statistical difference (P<0.05).4. In the sixth month after intervention, the comparions of the proportions betweentwo groups were carried out in exercise, accepting the sunshine, taking calcium,vitamin D supplements, consumption of milk and soy products and drinking strong tea,there were statistical difference (P<0.05).Conclusion1. The protection motivation intervention could be used to effectively reduce patients’fall risk, promote their knowledge and self-efficacy about osteoporosis and improve their health-related behaviors.2. The protection motivation intervention may be applied and extended in the clinicalnursing, because of its effective and feasible for reducing the risk of refractures.
