Objective This study is to evaluate the efficacy ofMicroRNA423-5p combined with BNP testing on the diagnosis of heartfailure. Furthermore, this study is also to find the potential cardiacdysfunction in patients with pacemaker implantation byMicroRNA423-5p testing.Methods A total of62patients were recruited and divided into3groups: group A(patients with heart failure), group B(patients withnormal heart function),and group C (patients with pacemakerimplantation). Expressions of BNP and MicroRNA423-5p、UCG were completed. Correlation analysis were initiated amongparameters of UCG、 pacing site and expression ofMicroRNA423-5p.Results Compared with group B and C, expressions of BNP andMicroRNA423-5p in group A were simultaneously increased (P<0.001). in comparison to groups B, expressions of BNP andMicroRNA423-5p in group C were increased significantly (P <0.001). Diagnostic efficacy of EF, BNP, MicroRNA423-5p on heart failurewere analyzed by the ROC curve, AUC area under the curve and95%confidence intervals of the three parameters were0.876(0.753,0.998),1.000(1.000,1.000),0.991(0.968,1.000)respectively. Result ofcorrelation analysis showed that in the pacemaker group,MicroRNA423-5p expression levels was positively correlated with leftatrial diameter, left ventricular end-systolic diameter, and leftventricular end-diastolic diameter (r=0.359, P=0.044; r=0.513, P=0.003; r=0.623, P <0.001), and negatively correlated with leftventricular ejection fraction (r=-0.715, P <0.001),and not correlatedwith the diameter of right ventricular and right atrium, thickness ofventricular septum.(P=0.437, P=0.307, P=0.714)Conclusion The expressions level of MicroRNA423-5p andBNP were positively correlated with NYHA class and negativelycorrelated with EF. Combined application of MicroRNA423-5p andBNP possibly improve the sensitivity of heart failure diagnosis.elevated BNP and MicroRNA423-5p expression levels in patients withpacemaker implantation may be associated with some degree of cardiacdysfunction associated with pacing, and its clinical significance wouldbe worthy of further study.
