Objective:Coronary heart disease,or coronary atherosclerotic heart disease,refers to a cardiovascular disease in which atherosclerotic changes in the coronary arteries cause stenosis of the lumen,leading to myocardial cells ischemia and hypoxia,and finally to myocardial ischemia and even necrosis.With its rising prevalence and mortality,the requirements for disease prevention,diagnosis,treatment,and prognosis have also increased.Early diagnosis and treatment are also the focus of current research.In recent years,miRNA has gradually entered people's field of vision and has been widely studied.Studies have found that miRNA-489 is closely related to coronary heart disease.In this experiment,the clinical significance of miRNA-489-3p expression in patients with unstable angina pectoris,acute myocardial infarction,and coronary heart disease was investigated by studying the expression level of miRNA-489-3p in patients' plasma.method:1.Collect 62 patients with coronary heart disease who were admitted to the Second Hospital of Jilin University from September 2019 to January 2020 and confirmed by coronary angiography or coronary CT angiography,including 20 cases of unstable angina pectoris and 20 cases of acute myocardial infarction.There were22 cases of heart failure group which the patients' basic disease is coronary heart disease,and 19 cases of control group with normal coronary angiography or coronary CT results(which has ruled out diagnosis of coronary heart disease)were selected.All subjects includingAge,gender,triglycerides,total cholesterol,LDL-C,apo(A1),apo(B),free fatty acids,,lipoprotein a,uric acid,previous medical history,such as history of hypertension,,etc.were collected.The patient collected venous blood in the early morning on an empty stomach.The plasma was separated by centrifugationwithin 1 hour at room temperature or stored at 4 ° C for 2 hours.The plasma was collected in an enzyme-free EP tube and stored in a-80 ° C refrigerator.The expression levels of miRNA-489-3p in plasma of each group were analyzed by RNA,reverse transcription and qRT-PCR.2.Statistical analysis was performed using SPSS statistical software,and graph analysis was performed using GraphPad Prism software.The measurement data are mean ± standard deviation(X ± SD),and the comparison between the mean arrays uses independent sample t test.When P <0.05,the difference was considered statistically significant.result:1.The relative expression of miRNA-489-3p in plasma in heart failure group which the patients' basic disease is coronary heart disease(0.4574±0.0794)was significantly lower than the control group(1.0938±0.4572),and the difference was statistically significant(P <0.001)2.The relative expression of miRNA-489-3p in plasma in the unstable angina pectoris group(0.6836±0.3209)and the relative expression of the acute myocardial infarction group(0.7151±0.3197)were lower than those in the control group.<0.05)3.There was no significant difference in the expression level of miRNA-489-3p between unstable angina pectoris group and acute myocardial infarction group(P>0.05).4.The expression level of miRNA-489-3p in heart failure group was lower than that in unstable angina group and acute myocardial infarction group,the difference was statistically significant(P<0.05).5.Perform ROC analysis on the predictive value of miRNA-489-3p in coronary heart disease,the heart failure group: we use 1 / miRNA-489-3p to draw ROC curve AUC = 0.75(95% CI: 0.65-0.85),the most The best cut-off value is 1.8399,the sensitivity is 90.9%,and the specificity is 67.8%.The acute myocardial infarction group: we use 1 / miRNA-489-3p to make the ROC curve:1 / miRNA-489-3p has an AUC of 0.465.(95% CI: 0.32-0.61),the best critical value is 1.7957,the sensitivity is35.0%,and the specificity is 54.1%.6.Correlation analysis using spearman method showed that the relative expression of miRNA-489-3p in coronary heart disease group with heart failure had no correlation with BNP and EF.conclusion:1.The expression of miRNA-489-3p in plasma of the patients with heart failure whose the basic disease is coronary heart disease is significantly reduced,and its sensitivity and specificity are high,suggesting that miRNA-489-3p may play an important role in the process of heart failure.2.The expression level of plasma miRNA-489-3p in the group of unstable angina pectoris and acute myocardial infarction is reduced,but the specificity is not high.It needs further research in the diagnosis and treatment.
