Objective:To definite the difference of cognitive performance between patients with obstructive sleep apnea and hypopnea syndrome and matched healthy controls. Further explore possible differences in peripheral levels of ApoA1and leptin between patients and controls, and investigate the correlation of these biomarkers with cognitive function.Methods:In this case-control study, a full-night polysomnography and a comprehensive neuropsychological assessment were administered on subjects. These sensitive neuropsychological tasks include Montreal cognitive assessment scale(MoCA), Wechsler adult memory scale(WMS), Digit symbol test(DST), Wisconsin Card Sorting Test(WCST), Stroop color-word test(SCWT) and N-back test.Fasting serum ApoAl and leptin levels were determined according to ELIS A method.Results:(1) All enrolled50patients and26controls.Compared with control group, OSAHS patients exhibited deficits in MoCA, WMS, WCST, SCWT, N-back, although DST were not.(2) In patients group, associations between MoCA, WMS, SCWT performance and sleep-breathing parameters were marginally significant.(3) Compared with control group, patients presented significantly higher levels of leptin and lower levels of ApoAl. Associations were found between these biomarkers and sleep-breathing parameters.The levels of ApoAl of the patients were significantly negative correlated with the levels of leptin.(4) In patients group, significant positive correlations were found between serum ApoA1levels and MoCA, WMS performance, and significant negative correlations were found between serum leptin levels and MoCA, WMS performance, also the stroop interfere time were positively correlated with serum leptin levels.Conclusions:The present study confirmed that male patients with OSAHS have widespread cognitive dysfunctions, differences in memory and executive(shifting, inhibition, updating) function were obviously significant, and is the first to use computer-based neuropsychological evaluation software to determine the impairments of executive function in patients with OSAHS. Reduced ApoAl and elevated leptin levels may have an important role in the pathogenesis of neurocognitive impairments in male patients with OSAHS. The detailed interaction needs further investigation.
