BACKGROUNDHealth care for pregnant women is an important measure to reduce maternal and child mortality and improve birth quality. Pregnancy care, particularly early pregnancy care is one of the key elements, it can early detection, diagnosis and screen high-risk pregnancy, pregnant women at high risk are able to access systematical monitor.Since1990, with the influx of foreign population to Shanghai, the number of births among migrant population in Shanghai growed rapidly. After that, the maternal mortality, perinatal mortality and detection rate of birth defects were all significantly increased compared to those of local people, which is a serious challenge to the maternal and child health, especially to the maternal care of Shanghai.Our study are trying to use the inventory survey to find out the health care of both local and migrant women during pregnancy in Xuhui district, identify the main factors which are affecting the early pregnancy care of migrant women, and propose relative policies and health management strategies.OBJECTIVEUnderstand the health care of migrant women during early pregnancy in Xuhui district, analysis the main influencing factors and propose improvement strategies.METHODS1.Quantitative survey:By using3self-designed questionnaires,27medical workers dealing with women’s health among13community health service centers of Xuhui district,500migrant women who brought their children to physical examination for the first time,650pregnant women who attended the community pregnant schools were interviewed. The contents were about the maternal health-care services, basic situation of women who established maternal health manual, the knowledge of early pregnancy care, health habits, early pregnancy care and early pregnancy health of migrant women.2. Qualitative survey:By using self-designed semi-structure questionnaires,16pregnant women who established maternal health manual,10migrant women,4medical staffs in maternity hospitals,2staffs of maternal and child health management and administration were in-depth interviewed. The contents including early pregnancy care, influencing factors and countermeasures.RESULTS1. The rates of establishing maternal health manual and Systematical management among migrant women are significantly lower than those of local women at the same time (P<0.01), the health care of migrant Pregnant women is the focus and difficulty of Pregnant women health care in our district. The rate of establishing maternal health manual is the most unequal points between local and migrant women.2."I don’t know","unnecessary" are the main reasons why migrant women didn’t establish maternal health manual. Education level, occupation, the time of coming to Shanghai, different types of insurance may be the main factors which affect migrant women accessing the knowledge of early pregnancy health care and information. Within the first3months during pregnancy is the best time to propagandize early pregnancy care.3. The differences of accessing knowledge is the point of local and migrant pregnant women.4. The rate of establishing maternal health manual during these two years dues to the screening of community pregnancy.5. The service of early pregnancy care in community health service center is appropriate. It can meet the needs of early pregnant care with the forms, equipments, time, skills and medical workers of services.
