Objective: To explore plasma HMGB1levels as well as therelationship with dysregulated glucose and lipid metabolism, insulinresistance (IR) and low grade inflammation(LGI) in health, obesity andnewly diagnosed type2diabetes mellitus (T2DM) subjects.Methods:155participants of Han race were recruited fromChongQing, China.80newly diagnosed T2DM patients admitted to ourhospital from February to November in2013were selected as T2DM groupand75healthy people were selected as NGT group. The diagnoses ofT2DM were based on the diagnostic criteria of World Health Organization(WHO) in1999. Both of them were divided respectively into twosubgroups: normal weight (NW) subgroup and obesity (OB) subgroup.Obesity was defined according to the World Health Organization-WesternPacific Region diagnostic criteria (2000) as body mass index (BMI)≥25kg/m2.Body fat parameters were calculated and BMI while waist-to-hipwere evaluated as well as the levels of fasting plasma HMGB1and IL-6were measured by ELISA.We have also detected fasting plasma glucose (FPG),2h plasma glucose (2hours after75g OGTT test,2hPG), HbA1c,fasting insulin (FINS), and lipids laboratory indicators. Pancreas beta-cellfunction and insulin resistance (IR) were assessed by homeostasis modelassessment (HOMA).Result: Plasma HMGB1levels were significantly higher in T2DMgroup than in NGT group(172.08±134.40) vs(128.59±107.90)ng/mL,P<0.05). Within groups of T2DM and NGT, plasma HMGB1levels in OBsubgroup were significantly higher than those in NW subgroup(136.96±116.51)v(s112.76±97.13)ng/mL,P<0.01and(207.20±143.13)vs(143.19±116.31)ng/mL,P<0.05while in T2DM-OB subgroup weresignificantly higher than those in NGT-NW subgroup(P<0.05). Bivariatecorrelation analysis showed that the plasma HMGB1levels were positivelycorrelated with waist circumference, BMI、FPG、2hPG、HbA1c、IL-6、FINS、HOMA-IR、 AST、TG(r=0.401~0.770,P<0.01or P<0.05),and were negatively correlated with HOMA-β (r=-0.518,P<0.01).Multiple linear regression analysis showed that HbA1c、TG、BMI、HOMA-IR、HOMA-β、IL-6were independently related to plasma HMGB1levels(P<0.01or P<0.05).Conclusions:Soluble HMGB1levels were increased respectively inobesity and type2diabetes patients,may be participating in the genesis anddevelopment of obesity and T2DM.
