Effect Of Diabetes Health Management In Rural Community Of Changsha City

Objective:To research and evaluate the current situation and effects of diabetes health management in rural community, to explore the relevant factors influencing the quality and effect of management, and to propose suggestions and countermeasures.Methods:In this study, questionnaire survey and qualitative interview were used. Questionnaire survey included the questionnaire of community health service centers, the managed diabetes patients and the chronic disease managers. In qualitative interview, the chronic disease managers were interviewed separately.Results:All the20community health service centre selected established public health department, and all were equipped with full-time chronic disease managers.The rate of diabetes health management was24.8%(14157/56992), and that ranged from13.9%-36.0%among the community health service centres Among the350managed diabetes patients selected,345(98.6%) were investigated; the rate of diabetes standardized-management was16.2%(56/345); the average score of health knowledge was21.41(the total score was100), and that of self-management was21.93(the total score was42); the control rate of blood glucose was29.0%(100/345); the average score of health service satisfaction was34.49(the total score was45),and among the factors of service satisfaction, the satisfaction rate ranged from40.7%-89.9%,and the satisfaction rate of the management effect was40.7%(140/344) which was the lowest; the average score of diabetes health knowledge, self-management,and health service satisfaction were all higher in patients who were managed standardly (32.53,23.61,36.77) than that in patients who were managed non-standardly (20.43,21.61,34.05). In terms of the40chronic disease managers,42.5%(17/40) had more than junior or below education;67.5%(27/40)worked on management and intervention for diabetes for more than three years;92.5%(37/40)attended relevant training of health management for diabetes;12.5%(5/40) were satisfied with the job.Conclusion:In rural community of Changsha city, the management organizations of diabetes were complete, but the rate of diabetes health management was low, the managing was not normative, and the effect of the management was unsatisfied.The chronic disease managers had relatively low academic degrees and low job satisfaction.
