Background and objectiveAtherosclerosis are a series of circulatory system disease which involving large elastic type (such as the common carotid artery) to medium-sized elastic type (such as epicardial coronary artery) artery intima systemic diseases。Coronary artery disease now have the high morbidity and mortality in our country, the increase of visceral adipose tissue is recognized as an important risk factor for the development of atherosclerotic cardiovascular disease, Epicardial adipose tissue is the true visceral fat deposited around the heart. Epicardial adipose tissue is an organ with endocrine function, can secrete several biologically active cytokines and inflammatory factors, and there is no fiber among epicardial adipose tissue, coronary artery and myocardial makes these cytokines can impact on coronary artery and myocardial.The carotid artery has been identified as the " window" to reflect the main artery systemic artery atherosclerosis,because it’s the import artery to supplies the head and neck,and The superficial location for conveniention to detect, Recent years the proposed of echo tracking (ET)technology have giving us a non-invasive, accurate ultrasound vascular function evalution tool, to evaluation of carotid artery wall elasticity changes in patients with coronary heart disease. The main purpose of this study is to explore whether the association exist between epicardial fat thickness and carotid artery elasticity change in the patients with coronary artery disease, and their relationship with coronary artery disease.MethodsAccording to coronary angiography, divided100subjects into4groups by whether there is coronary artery stenosis and the extent of the disease, group A (Non-CAD group), group B (1-vessel disease group),group C (2-vessel disease group), D (3-vessel disease group), each group have25subjects. Each subject was mearsured EFT and carotid ET, EFT was measured on the free wall of the right ventricle in the parasternal long-and short-axis views, and at the level of the right common carotid artery for assessment of intima-media thickness (IMT), and ET-related parameters. Analysis the relationship between ET-related parameters and coronary artery disease, EFT and coronary artery disease, and the correlation between EFT and ET-related parameters.Results1、EFT in CAD group was significantly higher than that of non-CAD group (CAD group was6.45±2.52mm, non-CAD group was2.59±1.02mm, P<0.05).2、EFT increase with the increase of the extent of coronary lesions.(B group,4.41±1.51mm, C group,6.27±2.16mm, the D group8.54±1.97mm, p<0.05)3、ET-related parameters:β,EP, PWV-βincreased and AC reduced with the increase of the extent of coronary lesions, AI has no clear trend.4、EFT is positively correlated with carotid IMT, β, Ep, AI, and PWV-β (r=0.517、0.422、0.542、0.494, p<0.05), negatively correlated with AC (r=-0.428, p<0.05), and there was no correlation between EFT, age, BMI, BP, BG, and BF. Conclusion1、Epicardial fat thickness increased and the elasticity of carotid artery decreased with the increase of the extent of coronary artery lesions. 2、Epicardial fat thickness and carotid artery elastic changes have good correlation.in patients coronary heart disease.3、Epicardial fat thickness and ET-related parameters of carotid artery are both important predictors of coronary artery disease.
