Objective:To compare the clinical efficacy of closed prying reduction and open reductionwith fixation treatment for calcaneal fractures (Sanders Ⅱ-Ⅳ).Method:Collect40cases of calcaneal bone fractures included in the standard Sanders Ⅱ-Ⅳ who had been undergone surgery during February2012to February2014in theorthopaedic trauma ward of the first affiliated hospital of zhengzhou university,According to the doctor-patient communication groups voluntarily,patients wererandomly divided into treatment group and control group (n=20), the treatmentgroup by operation of open reduction and internal fixation with anatomic plate andthe control group through closed reduction and fixation by using the needle.Inpatients with calcaneal tubercle Angle (Bohler Angle), calcaneal crossing Angle(Gissane’s Angle) of data measurement, length of hospital stay and postoperativecomplications in patients with comparative analysis and statistical processing, tocompare the clinical curative effect by open reduction and internal fixation withanatomic plate and closed reduction and fixation with the needle in the treatment ofSanders Ⅱ-Ⅳ calcaneal fractures, to provide reference for clinical treatment.Results:40cases received a follow-up, the follow-up time during5-13months,8monthson average.Results show that the two methods of surgical treatment for Sanders Ⅱ-Ⅳ calcaneal fractures, both can obtain more satisfactory clinical efficacy.In Marylandscores for foot and the excellent score aspect,the treatment group was better thancontrol group (P <0.01),in terms of imaging anatomy of the angle,comparison andanalysis on the angle of the two groups of postoperative Bohler recovery (P>0.05),without statistical differences.the recovery of Gissane’s Angle (P <0.01)between the two groups,with statistically significant differences.two groups ofpostoperative complications occurred in different degrees, comparative analysisfound no significant difference (P>0.05).Two groups had significant difference on the postoperative hospitalization time (P <0.05), the control group was shorter thanthe treatment group.Conclusion:Two methods of surgical treatment for Sanders Ⅱ-Ⅳ calcaneal fractures canrectify calcaneal anatomic parameters close to normal,but treatment with openreduction and internal fixation, the curative effect was better than closed pryingreset.Under the premise of controlling the complications of postoperative incisionabnormal by open reduction and internal fixation, it can be used as a clinical preferredmethod,to improve the clinical curative effect.Closed prying reset had a small surgicaltrauma, fewer complications, shorter hospitalization time, etc, on the premise ofgrasping the indications strictly, can be an available option.
