Analyze The Factors That Influence The Healing Of Urethral Mucosa After Endoscopic Urethrotomy

ObjectiveTo discuss the factors that influence the healing of urethral mucosa afterEndoscopic urethrotomy,and to provide a certain basis for the clinical treatment ofurethral stricture.MethodsThis retrospective cohort study consisted of30urological patients who hadpost-urethral stricture in the Department of Urology of The First Affiliated Hospitalof Zhengzhou University from2009to2011.All patients were male,all patients werethrough retrograde urethrography and voiding cystourethrography before theoperation to know the length of strictures. Endoscopic urethrotomy wereoperated,three methods-pericatheter urethrography,urethroscopy,urine flow rate wereused after1month to observe the healing of urethral,appraise the relationshipbetween the indwelling time of catheter and urethral mucosa healing. ResultsThe indwelling time of catheter needed4to20weeks.25patients(83.3%)require more than12weeks,4patients(13.3%) require less than8weeks,1patient（3.3%) was failure and implemented a second surgery. The urine flow rate is equalor greater than15ml/s:1patient（3.3%) after4weeks;4patients after8weeks;24patients after12weeks;27patients after16weeks;29patients after20weeks,thedifference is statistically significant(P<0.001). The patients have been followed up for24months,20patients(66.7%) of them achieved a satisfactory result,and the urineflow rate is equal or greater than15ml/min;9patients（30%) of them achieved apreferable result,need to regular urethral dilation each6months;1patient（3.3%）formed re-stricture after remove the catheter,and implemented a second surgery andthen lost to the follow up.Conclusions1The time of the indwelling catheter is positive correlated with urethralhealing and treatment effect.2In the situation of strengthen the patient’s nutrition and control theinfection,the best time of indwelling catheter removal after endoscopic urethrotomyis8weeks to12weeks.The withdrawal of the stenting catheter should be guided bypericatheter urethrography,urethroscopy, urine flow rate until healing up of thedisruptured urethra.
