Objective: To observe the effects of Xinshuaikang and itsdisassembled prescriptions on general condition and urinary volume inchronic heart failure (CHF) rats. Determination of heart function andurinaryaquaporin2water channel protein(AQP2),hypothalamic argininevasopressin (AVP) in rats with chronic heart failure. To investigate theXinshuaikang and its disassembled prescriptions impact on watermetabolism in rats with heart failure.Methods: Selecting Wistar rats weighing about250g, making theanimal model of chronic heart failure with coronary artery ligation, after4weeks operation by echocardiogram examination, the heart failure rats withleft ventricular ejection fraction below45%were included in theexperiment. randomly divided into6groups respectively, which were modelgroup, Xinshuaikang group, tonifying Qi group, warming Yang group,promoting blood circulation group,removing water group, and setting shamoperation group. Xinshuaikang group received gastric perfusion ofXinshuaikang, tonifying Qi group, warming Yang group, promoting bloodcirculation group,removing water group were given tonifying Qi, warmingYang, promoting blood circulation, removing water durgs of Xinshuaikangby gavage respectively. Model group were given the same dose of ultrapure water by gavage. Sham operation group with ultra pure water bygavage as blank control group. The experiments were divided into2batches,the first batch of rats were given durgs for4weeks, the second batch of ratswere given durgs for8weeks. Observing animal’s behavior and the urinevolume, using hematoxylin-eosinstaining (HE staining) to observe thepathological changes in the heart and kidney, using double antibodysandwich ELISA method to detect AQP2concentration in urine of rats with heart failure.using real time PCR method to detect the expression of AVP inhypothalamusResult:1. Animal behavior and24hour urine volume of rats in each groupwere observed: The treatment group compared with the model group, all canimprove the general condition of rats; After4weeks of drug intervention,compared with the tonifying Qi group, warming Yang group and promotingblood circulation group, there were differences in diuretic effects ofremoving water group. After8weeks of drug intervention, Removing watergroup compared with other disassembled prescriptions had no difference inurine volume, and the removing water group given drugs for8weekscompared with4weeks increased urine output amplitude than the other eachdrug intervention group; Xinshuaikang group,tonifying Qi group, promotingblood circulation group, removing water group compared with the modelgroup, there was statistically significant difference (P<0.05), Xinshuaikangand diuresis medicine in Xinshuaikang disassembled prescriptions had thediuretic effect is the most obvious in heart failure rats.2. Echocardiogram evaluation: After4weeks of drug intervention inrats with heart failure, compared with the model group rats, the rats ofXinshuaikang group and tonifying Qi group, promoting blood circulationgroup were significantly higher in LVEF (P<0.05), the Xinshuaikang groupwas most obvious, and warming Yang group, removing water group rats’LVEF increase of no significant difference (P>0.05). Rats for8weeks,compared with the model group rats, Xinshuaikang group, tonifying Qigroup, promoting blood circulation group, warming Yang Group andremoving water group’s left ventricular ejection fraction was significantlyincreased (P<0.05), showed that Xinshuaikang and its disassembledprescriptions can improve cardiac function in rats with heart failure aftermyocardial infarction, among which the most obvious Xinshuaikangcovered. And along with the time extension, warming Yang and removingwater disassembled prescriptions also can improve left ventricular ejectionfraction, but this effect is a little bit weake than tonifying Qi drugs andpromoting blood circulation drugs.3.HE staining was used to observe pathological changes in heart and kidney: compared with the model group, Xinshuaikang group, promotingblood circulation group, tonifying Qi group had the effect of improving thecardiac pathology in rats with heart failure; Xinshuaikang group, tonifyingQi group, promoting blood circulation group,removing water group andwarming Yang Group had the effect of improving the renal pathology in ratswith heart failure.4weeks group compared with8weeks group, along withthe time extension, Xinshuaikang group, tonifying Qi group and promotingblood circulation group had the improvement in cardiac and renalpathological changes was strengthening.4.Detection of urinary aquaporin2: After4weeks of drug interventionin rats with heart failure, water channel protein2in urine of rats wasdecreased compared to the model group,and the effect of Xinshuaikang wasthe most obvious, removing water group followed, which was differencescompared to the other parties split the group; while after8weeks drugintervention, Xinshuaikang group had the same effect which was the mostobvious,while the effect of removing water group was similar to tonifyingQi group, warming Yang group and promoting blood circulation group (P>0.05).This showed that along with the time extension, the inhibitory effectof tonifying Qi drugs, warming Yang drugs and promoting blood circulationdrugs on the water channel protein were gradually increased, while therewere no significant changes in removing water drug. Contact to the index ofheart function, this result is related to improving heart function with thelong-term using, improving renal blood supply, which have an impact onwater metabolism.5.Determination of hypothalamic arginine vasopressin (AVP) mRNAby real time fluorescent quantitative PCR: After4weeks of drug interventionin rats with heart failure,the hypothalamic arginine vasopressin (AVP)mRNA expression: Xinshuaikang group, tonifying Qi group, promotingblood circulation group, removing water group compared with the modelgroup, there was statistically significant difference (P<0.05), showed thatXinshuaikang and tonifying Qi, promoting blood circulation and removingwater group drugs of its disassembled prescriptions can reduce theexpression of hypothalamic arginine vasopressin (AVP) mRNA, The effectof the Xinshuaikang group and removing water group were the most obvious, compared with tonifying Qi group, warming Yang Group and promotingblood circulation group had difference.And after8weeks treatment,Xinshuaikang group and tonifying Qi group,Warming Yang group,promoting blood circulation and removing water group had the same effectwhich could reduce the expression of the rat’s hypothalamic argininevasopressin (AVP) mRNA, and there were no difference among the groups.Combined with the analysis of the results, Xinshuaikang and itsdisassembled prescriptions could decrease the expression of hypothalamicAVPmRNA and effect of renal AVP receptor. Then effect on renal AQP2soas to further increase the urine volume of rats with heart failure.Conclusion:1. Xinshuaikang and its disassembled prescriptions could improve thegeneral situation of heart failure rats, and reduce the degree of myocardialinjury after myocardial infarction, reduce renal tissue damage, improve leftventricular ejection fraction, could improve cardiac function in heart failurerats and with the prolong drug intervention time, this effect is moreobvious.2. Xinshuaikang and its disassembled prescriptions could reduce theconcentration of urine aquaporin2water channel protein, along with thetime extension, the inhibitory effect of tonifying Qi drugs, warming Yangdrugs and promoting blood circulation drugs on aquaporin2water channel protein were gradually increased.3.Xinshuaikang and its disassembled prescriptions in the treatment ofchronic heart failure was related to improve cardiac function, reduce theexpression of AVPmRNA in the hypothalamus, which increase the urinevolume.4. For animal model with chronic congestive heart failure rats,usingtonifying Qi medicine alone composed with red ginseng and astragalusmongholicus can increase the amount of urine of rats with heartfailure,compared with the model group, the concentration of aquaporin2water channel protein in urine was lower, and its mechanism may be relatedto decrease the expression of hypothalamic AVPmRNA.5. For animal model with chronic congestive heart failure rats,Usingwarming yang medicine alone composed with aconite and cassia twig can increase the amount of urine of rats with heart failure,compared with themodel group, the concentration of aquaporin2water channel protein inurine was lower, and its mechanism may be related to decreasethe expression of hypothalamic AVPmRNA.6. For animal model with chronic congestive heart failure rats,Usinghemorheologic agent alone composed with red flower,the root of red-rootedsalvia and radix paeoniae rubra can increase the amount of urine of ratswith heart failure,compared with the model group, the concentration ofaquaporin2water channel protein in urine was lower, and its mechanismmay be related to decrease the expression of hypothalamic AVPmRNA.
