ObjectiveSepsis is a common occurrence in critical ill patients. The incidence rate and mortality rate is high. Pathogenesis of immune dysfunction in sepsis is a research hotspot in recent years. The study was to observe the clinical curative effect of electroacupuncture at Zusanli and Guanyuan for the treatment of the patients with sepsis and explore the improvement of clinical symptoms and prognosis of immune regulation mechanism, and provide a theoretical basis for the Chinese medicine treatment of the patients with sepsis.Methods58cases were randomly divided into control groups (29cases) and treatment groups(29cases). The control group was given conventional western medicine treatment and the treatment group was given electroacupuncture at Zusanli and guanyuan based on conventional therapy of western medicine. Two groups were observed leukocyte, neutrophil, ESR, Oreaction protein, procalcitonin, lactic acid, albumin, globulin, APACHEII score, in patients with ICU hospitalization,28day prognosis, detection of T cell subsets (CD3+, CD4+, CD8+, CD4+/CD8+) and monocyte human leukocyte antigen DR (HLA-DR).Results1.Inflammatory markers (leukocyte, neutrophil, ESR and Oreaction protein). Leukocyte, neutrophil, ESR and C-reaction protein were no significantly different between the two groups before treatment (P>0.05), which of treatment group significantly decreased at third day and seventh day(P<0.05). There was significant difference between treatment group and control group(P<0.05). 2. Procalcitonin(PCT). The level of PCT of two groups were not significantly different(P>0.05), which of treatment groups significantly decreased at third day and seventh days(P<0.05).3. Lactic acid (LAC). The level of LAC of two groups were not statistical significantly different (P>0.05). After treatment, the level of LAC of two groups decreased, which of the treatment group decreased significantly at seventh day (P<0.01), which was statistically significantly different compare with the control group (P<0.05).4. APACHE-Ⅱ score. The APACHE-Ⅱ score of two groups was not statistically significant different before treatment (P>0.05). After treatment, The APACHE-Ⅱ score of two groups significantly decreased at third and seventh days (P<0.01), which of the treatment group were significantly different compared with control group (P<0.05).5. Time in ICU and outcomes at28days. The time in ICU of treatment group was significantly shorter than that of control group (P<0.05). Outcome of treatment group is better than the control group without significant difference at28days(P>0.05).6. T cell subsets. There was no significant difference in level of T cell subsets between two groups (P>0.05). The level of CD3+and CD4+and the ratio of levels were rase, which of treatment group increased significantly at seventh days after treatment(P<0.01). There was significant difference compared with the control group (P<0.05)。After treatment, Level of CD8+of treatment group decreased, while which of control group increased, there was significant difference in control group at seventh day (P<0.05). There was significant difference between treatment group and control group at seventh days (P<0.05).7. The level of HLA-DR. There was not significant difference in the level of HLA-DR between two groups (P>0.05), which of two groups increased after treatment, and that of the treatment group were significantly different at seventh day (P<0.01). There was significantly different between two groups (P<0.01).ConclusionEffects of electroacupuncture at Zusanli and Guan yuan could reduce septic inflammatory action, the level of lactic acid procalcitonin, improved APACHEII score, reduced hospitalization of ICU. Through the detection of T cell subsets and HLA-DR level, we found it could improve CD3, CD4, and HLA-DR levels, reduced CD8levecls. The clinical effects of electroacupuncture at Zusanli and Guan Yuanxue showed that it could improve immune status and correct the immunity disorder of septic patients, so as to achieve the efficacy in the treatment of the patients with sepsis. It provide a theoretical basis for the Chinese medicine treatment of sepsis.
