The Study On The Mechanism Of Clinical Effect Of Yishen Qingli Tongluo Xuewei Maixian Decoction, The Formula For Tonifying Kidney, Clearing Collaterals Acupoint Catgut Embedding Therapy On Chronic Pelvic Pain Syndrome

Objective: Investigation of the clinical curative effect and the mechanism of YishenQingli Tongluo Xuewei Maixian Decoction, the formula for tonifyingkidney, clearing collaterals acupoint catgut embedding therapy on chronic pelvic painsyndrome, through the method of clinical trials.Methods:180patients being up to the standard were randomly divided into theembedding therapy group (90cases) and the western medicine group (90cases),according to the random number table method (1:1).The western medicine group weregiven tamsulosin hydrochloride capsules,0.2mg, QD, Po; Indometacin SustainedRelease Tablets,25mg, Tid, Po. The catgut embedding group were treated by acupointcatgut embedding, the acupuncture points Qugu(CV2), Shenshu(BL23),Zhibian(UB54), Huiyin(RN1), Sanyinjiao(SP6) were chosen,embedding once everytwo weeks, four weeks as a course, two courses of a treatment. Observe the changesof TCM symptom score, NIH-CPSI score (NIH Chronic Prostatitis SymptomIndex),the number of prostatic fluid lecithin corpuscles, SAS (Self-Rating AnxietyScale) and SDS scores (Self-Rating Depression Scale),plasma substance P (SP),plasma β endorphin (β-EP).Results:○110cases in western medicine group were exfoliated, among the rest80cases,23cases were cured;19cases had remarkable effects; and21cases had effects.10cases in catgut embedding group were exfoliated, among the rest80cases,36caseswere cured;25cases had remarkable effects; and12cases had effects, according tostatistical comparison of Z=-5.803, P=0.000, catgut embedding therapy group is betterthan western medicine group. The total efficiency of western medicine group was78.75%and the catgut embedding group was91.25%.Compared the total efficiency oftwo groups x2=4.902, P=0.027, catgut embedding therapy group is better than westernmedicine group.○2Compared the TCM syndrome score of both groups after treatment: WM group was9.41±2.68, embedding group was7.11±2.41, compared the two groups:t=-5.577,P=0.000, embedding group is clear better than western medicine group.③Compared the lecithin after treatment: WM group was26.26±8.03, embeddinggroup was31.24±5.41, compared the two groups:t=4.597, P=0.000, embeddinggroup is better than the medication group.④Compared the NIH-CPSI total score andpain score after treatment: WM group were13.83±5.18,5.35±1.81, embedding groupwere10.09±2.70,3.51±1.78, the comparison of two groups:t=-5.652,-6.179, P=0.000,0.000,embedding group was obvious better than western medicine group.⑤Compared the SAS and SDS scores after treatment:western medicine group were38.32±5.85,37.50±4.54, embedding group were35.48±3.61,35.31±5.78, thecomparison of the two groups:t=-3.870,-2.349, P=0.000,0.010, embedding group wasclear better than western medicine group.⑥Compared the SP after treatment:westernmedicine group were778.64±86.96, embedding group were715.60±85.10, thecomparison of the two groups:t=-4.750, P=0.000, embedding group was better thanwestern medicine group.⑦Compared the β-EP after treatment, western group were224.44±12.66, embedding group was254.12±15.84, the comparison of the twogroups: t=12.363, P=0.000, embedding group was better than western medicinegroup.Conclusion: Yishen Qingli Tongluo Xuewei Maixian Decoction, the formula fortonifying kidney, clearing collaterals acupoint catgut embedding therapy cansignificantly relieve the clinical symptoms of the patients with chronicpelvic painsyndrome at the stage of Kidney Insufficiency and dampness and heat stasis,improvethe anxiety and depression of the patients, increase the number of prostatic fluid inlecithin corpuscle. The content of β endorphin in plasma of the patients withchronicpelvic pain syndrome was decreased obviously, the substance level of PlasmaP was increased obviously. The content of β endorphin in plasma was increased andthe substance level of Plasma P was decreased of the both groups aftertreatment,there was statistical significance compared with prior treatment. And thecatgut embedding group was better than the control group. Suggesting that they maybe involved in inflammation of chronic pelvic pain syndrome.
