1Objective Through retrospective analysis and comparison, analyze clinicalcurative effect and clinical pragmatism in removing loose body under arthroscopy and usingbioabsorbable cartilage screws treating in its early period provide realistic basis for selectingwhich way for clinical treatment on patella osteochondral fracture.2Methods Collect patella osteochondral fracture patients with complete clinicaldata, who require surgical treatment in our hospital during January2010to March2013, allpatients are closed fracture. MRI suggest that all patients have patella osteochondral fracture.At first,search and cartilage damage areas under arthroscopy,6cases remove loose bodyunder arthroscopy and the other15cases use bioabsorbable cartilage screws. Drainage tube isremoved within24hours after internal fixation. Antibiotic prophylaxis for surgical siteinfection is no more than24hours. After two weeks the patient start flexion and extensionfunctional exercise of the knee under the protection of knee hinge brace fixation.After fourweeks the patients walk with part weight bearing under knee hinge brace fixation. Six to eightweeks after the operation, the patients walk full weight bearing under knee hinge bracefixation without crutch. Record in details, and follow up knee joint pain, function, motion inthe time of pre-operation,1month after operation,3months after operation, postoperative6months, postoperative1year. Then evaluate according to internationally recognized Lysholmand Tegner standard for evaluation. To each group of data, adopt SPSS18.0software and Ttests of different treatment methods in the same period to carry out statistical analysis comparison, marking measuring data asX±S. If P <0.05, it is statistically significant,confirming the early clinical effect of single rivet and double-row rivets treating rotator cuffinjury.3Results Results from Lysholm score and Tegner score are basically thesame, further proving that the research is scientific.Compare according to Lysholm scoring index for the treatmen of the patellaosteochondral fracture patients,the comparing P values of patients applying removing loosebody under arthroscopy and using bioabsorbable cartilage screws row at the same time aftersurgery are: pre-operation is0.591, postoperative one month is0.459,3months after surgeryis0.000,6months after operation is0.000,12months after surgery is0.000.Compare according to Tegner scoring index for the treatmen of the patellaosteochondral fracture patients,the comparing P values of patients applying removing loosebody under arthroscopy and using bioabsorbable cartilage screws row at the same time aftersurgery are: pre-operation is0.789, postoperative one month is0.505,3months after surgeryis0.001,6months after operation is0.000,12months after surgery is0.000.4Conclusions After evaluating knee joints of patients with patella osteochondralfracture through Lysholm and Tegner standard criterias, we find all preoperative P>0.05.This shows that knee joint function and disease of all patients have no obvious differencebefore operations, and we can go on the subsequent statistical analysis.For patella osteochondral fracture patients,compare through Lysholm and Tegnerstandard criterias in every periods, that one month later indicate that the treatment of the twooperation method has no difference in a month. After3months,6months,12months,all P <0.05indicate that there are clear differences using bioabsorbable cartilage screws is muchbetter than removing loose body under arthroscopy for treat patella osteochondral fracture.
