OBJECTIVE Questionnaires,physical examination,and laboratory tests are used to explore the train drivers’basic status of the cardiovascular health in Kunming Railway station;then reveal the cardiovascular health influence factors for train drivers.And to provide a scientific basis for the cardiovascular health prevention interventions and a theoretical basis for enhancing the health of train drivers’restructuring quality of life.METHODS This study was Cross-sectional survey. All train drivers in Kunming railway station were investigated between June and August,2012. Questionnaire were used to obtain train drivers demography characteristics.Occupational health examination and laboratory tests were used to obtain cardiovascular health statues.Combined with qualitative interviews.Finally,the corresponding application Epidata and Excel data entry,import SPSS data analysis.RESULTS1.Demographic characteristics:This study investigated a total of1967people,all males. And aged35to45were up to984(50.02%). The number of overweight was917(46.61%); central obesity and normal persons were1054(53.58%),913(46.42%). The number of mild, moderate and severe life stress were106(5.39%),1552(36.70%),309(42.91%).Occupational characteristics:this survey, types of train drivers into electric locomotive drivers and diesel locomotive drivers, the number was1212(61.62%),755(38.38%); persons over15years of seniority to1251(63.60%); one week the average working hours more than50hours for1568(79.72%);1348(68.53%) people were needed to wheel and night shifts;2.The basic indicators of cardiovascular conditions:1482people’s blood pressure were normal,485were abnormal,the abnormal rate were24.66%;1509people’s glucose were normal,458people were abnormal,abnomal rate were23.28%;1,800people’s lipids were normal,167people’s were abnormal,anomaly rate of8.49%;1784people’s heart rate were normal,183people were abnomal,anomaly rate were9.30%;1725people’s electrocardiogram were normal,242people abnormaly, abnomal rate were12.30%.3.Logistic regression analysis showed:influence factors of blood pressure were:Age (OR=2.96),educational level(OR=0.68),overweight(OR=2.94),central obesity (OR=4.33),smoking(OR=1.50),drinking(OR=1.15),jobs(OR=1.93),length of service(OR:2.43),working pressure(OR=4.32);Influence factors of glucose are: overweight(OR=1.42),life stress(OR=1.70),drinking(OR=1.24),working pressure(OR:1.42);lipid factors are Age(OR=1.92),life stress(OR=2.67), eating greasy food(OR=1.23),length of service(OR=1.71),work week(OR2.29),working pressure(OR=1.96),work-family conflict(OR=1.79);heart rate factors are:life stress(OR value=2.47),working pressure(OR=1.92); electrocardiogram factors are:overweight(OR=1.34),1ife stress(OR:1.80),work week(OR=1.63),working pressure(OR=1.94),occupational burnout(OR=1.34). Cardiovascular health factors were:age(OR=1.03),overweight(OR=1.98),central obesity(OR=1.61),smoking(OR=1.10),types(OR=1.46),seniority(OR=1.02), working pressure(OR=1.63),occupational burnout(OR=1.66).Multivariate logistic regression analysis showed:blood factors are:age,central obesity,smoking,type of work,work pressure;blood f-actors are:overweight(OR=1.53),life stress(OR value=1.28),alcohol(OR=1.26);lipid factors are;working pressure(OR=2.26),work week(OR=2.11),life stress(OR=2.06),1ength of service(OR=1.56),eating gteasy food(OR=1.23);heart rate factors are:life stress (OR=2.49),working pressure(OR=1.96);eJectrocardiogram factors are:working pressure(OR=1.97),"fe stress(OR:1.76),the work week time(OR:1.57). occupational burnout(OR=1.17).cardiovascular health influence factors are:central obesity(OR:1.71),smoking(OR=1.10),jobs(OR=1.29),working pressure(OR=1.30),burnout(OR=1.69). Principal component analysis showed:analysis extracted two principal components, the first principal component is occupational factors:stress, burnout and job; second main ingredient is central obesity and smoking. The first principal component reflects the impact of occupational factors, the second principal component reflects the impact of body weight and behavioral habits.CONCLUSION Train drivers cardiovascular health is not optimistic, the main factors are:work stress, life stress, work week, age, central obesity, overweight, seniority, smoking, drinking, eating greasy food. Which age, overweight, central obesity, smoking, drinking, eating greasy food belongs to the general population of the common risk factors for cardiovascular health, due to the special nature of the profession of train drivers, working pressure, working hours a week, length of service, are train drivers unique to the profession of risk factors for cardiovascular health, therefore, train drivers cardiovascular health need to intervention from the first take control of occupational hazards, but also to change unhealthy behavior, maintain a healthy weight. In order to effectively reduce the risk of cardiovascular disease by train drivers.SUGGESTIONS The train driver cardiovascular health interventions from two aspects:1. Occupational harmful factors interventions:according to the train driver a special professional group, improve operation space and actively promote reform of the system of flight, improve labor organization management, reasonable arrangement of night work of flight time, appropriately increase rest time, alleviate the fatigue of the driver and work pressure, focus on the older, longer length of train drivers, scientific intervention measures, to ensure the train driver’s physical health.2.General population interventions:Train drivers more than35years old, should be early detection, early diagnosis, early treatment; early interventions to enhance health education lectures, seminars, tutorials, health education bulletin boards, network communication methods such as health education. Change unhealthy lifestyles start. Exercise, reasonable diet, maintain a healthy state of mind, for the prevention of cardiovascular disease incidence of positive significance.
