Objective: To retrospectively analyze the relationship between course of disease, scopeof lesions, colonoscopic index, severity of mucosa biopsy and severity of disease, andthe clinical features in different scopes of lesions in patients with ulcerative colitis so asto offer theoretical basis for the clinical diagnosis and treatment of ulcerative colitis.Method:294patients with ulcerative colitis from the First Affiliated Hospital of DalianMedical University from1.1.2009to1.1.2014were collected in this study. Thediagnosis was made based on “the National Consensus on the Diagnosis and Treatmentof Inflammatory Bowel Disease” by the Digestive Disease Branch of Chinese MedicalAssociation. SPSS13.0software was used for the statistical analysis. The enumerationdata were described by percentage. The measurement data were measured bymeans±standard deviation. Analysis of variance Chi-square test with p<0.05wasconsidered statistical significance.Results:1. General data:168patients were male and126female. Male:female=1.33:1. Thepatients’ age was ranged between20and87years with an average age being45±15years. There were168patients (57.15%) with ages between30-49years, considered tobe the predilection age.2. Clinical features:diarrhea (239cases,81.29%), abdominal pain (234cases,79.59%),purulent bloody stool (171cases,58.16%) were the most common clinical features.Weight loss was observed in120cases (40.82%), fever in79cases (26.87%), anemia in90cases (30.61%), tenesmus in84cases (28.58%), mucus stool in60cases (20.41%),bloody stool in63cases (21.42%), and extra-intestinal manifestations in21cases (7.14%).3. Clinical types:chronic recurrent type was observed in208cases (70.75%) and initialtype was observed in86cases (29.25%).4. Relationship between endoscopic index, mucosa biopsy inflammation and severity ofdisease4.1Endoscopic index: mild, moderate and severe patients were classified accordingto the Rachmilewitz endoscopic index. The specific scores were420,478, and328inthe three groups of patients, respectively. Comparison between the three groups showeda statistical significance (p<0.05).4.2Inflammation severity of mucosa biopsy:122out of294patients underwent mucosabiopsy. The results showed that17mild cases (94.44%) and1moderate case (5.56%)was found in Grade I;11mild cases (24.44%),29moderate cases (53.33%), and10severe cases (22.23%) were found in Grade II; and16mild cases (29.63%),18moderate cases (33.33%), and20severe cases (37.04%) were found in Grade III.Comparison between the three groups showed a statistical significance (p<0.05).5.Scope of lesion and clinical characteristics:E1(Proctitis):bloody stool> diarrhea>abdominal pain>fever; E2(left colitis): abdominal pain> diarrhea> bloody stool> fever;E3(extensive colitis): abdominal pain> bloody stool> diarrhea> fever. The biggestproportion of patients with fever was found in E3(extensive colitis).6. The course and severity of disease analysis:126mild cases (63.64%),48moderatecases (24.24%) and24severe cases(12.12%) were found in0to5years of course;12mild cases (28.57%),24moderate cases (57.14%) and6severe cases (14.29%) werefound in5to10years of course;6mild cases (11.12%),24moderate cases (44.44%),and24severe cases were found in course over10years. Comparison between thethree groups showed a statistical significance (p<0.05).Conclusion:1. Predilection age in patients with UC was30-49years. Males are more frequentlyaffected than females.2. The main clinical manifestations include diarrhea, abdominal pain, mucus purulentbloody stool. The patients are likely to lose body weight in varying degrees and extraintestinal manifestation is rare. The distribution of lesions is mostly relevant to proctitis(E1); Chronic recurrent type is the main type of lesions.3. The severity of lesion is relevant to the course of disease, extent of disease, the endoscopic index, and the severity of mucosa inflammation.
