Objective To investigate the prognostic value of NT-proBNP and pulmonary artery systolic pressure for chronic left ventricular failure.Methods From142hospitalized left heart failure patients of The Affiliated Hospital of Medical College Qingdao University,we gather their age, gender, heart rate, body mass index and other general information, assessment their cardiac function with the NYHA class, and at the same time measure their NT-proBNP, sodium, creatinine, left ventricular ejection fraction, pulmonary artery systolic pressure, left ventricular end-diastolic diameter and other indicators, then follow-up cardiovascular events one year after discharged.Results concentrations NT-proBNP has significant positive correlation with pulmonary artery systolic pressure in142cases of heart failure in patients with chronic left heart failure,but it is not a simple linear correlation.The incidence of1-year endpoint events of patients with pulmonary artery systolic pressure≥50mmHg were significantly increased than patients with pulmonary artery systolic pressure<50mmHg, the difference was statistically significant (x2=17.045, p<0.05); and The incidence of1-year endpoint events of patients with NT-proBNP concentrations≥2218pg/ml was significantly higher than patients with NT-proBNP concentrations<2218pg/ml, the difference was statistically significant difference (x2=54.685, p<0.05); there was a significant difference of pulmonary artery systolic pressure, and plasma NT-proBNP concentration between endpoint events group and non-endpoint events group (P<0.01); Respectively using NT-proBNP and pulmonary artery pressure to predict one-year prognosis of chronic left ventricular failure, the area under the ROC curve was0.970(95%confidence interval:0.912-1.000, P=0.00),0.784(95%CI:0.691-0.878, P=0.00), the optimal boundary values were2225.5pg/ml and54.5mmHg, and As the cut-off point at the optimum cutoff value, the sensitivity were95.6%,75.6%and the specificity were72.2%,71.1%, positive predictive values were61.4%and54.8%, negative predictive values were97.2%and75.0%, and accuracy were79.6%and72.5%. Compare with pulmonary artery systolic pressure predicted value of NT-proBNP is higher.Conclusion pulmonary artery systolic pressure has significant positive correlation with NT-proBNP in Chronic heart failure patients with severe pulmonary hypertension. The incidence of cardiovascular events One year increase significantly in Chronic heart failure patients with severe pulmonary hypertension. The incidence of cardiovascular events One year increase significantly in Chronic heart failure patients with the level of NT-proBNP exceed2225.5pg/ml. NT-proBNP is a better predictive value than pulmonary artery pressure in the one year prognosis evaluation of patients with left heart failure.
