[Purpose]The study used the "modified early warning scoring system (Modified Early Warning Score)(hereinafter referred to as MEWS scoring system)" as the tool of emergency triage assessment and triage rapid,comparing with "acute physiology and chronic health evaluation (acute physiology and chronic health evaluation)(hereinafter referred to as APACHE Ⅱ scoring system)"and the existing means of "four rankings and three diagnostic distinction", combined with each other, appraise the accurate assessment of emergency department patients with disease severity, to identify " potential emergency critically ill patients,"appraise the truncation point, to make a reasonable disposal and diversion to patients in emergency department, limiting emergency room "entry " and improving part of the crowding of emergency department.[Method]Using MEWS score the8750patients of emergency department visits in one Hospital, using the best cut-off point of5points to retrospective analysis of the data, combined with the means of three districts and four fractionate in emergency triage to calculate the error rate of Triage calculated (hereinafter referred to as misclassification rate).Random the30simples in the emergency department of one Hospital. Using both MEWS score and APACHE Ⅱ score, comparative and analysis of the data, test the correlation of two score.[Result]1Completing the collection of8750cases datas, the average of age:46.06±18.25.4522cases are male,51.7%of the total number of cases.4228cases are female,48.3%of the total number of cases.2Emergency classification is divided into four rankings and three diagnostic distinction.Three diagnostic distinction including the red area, yellow area and green area. The patient’s condition in the red area is more serious then descending in order. Statistics show that the red area had a total1,013cases of patients, patients with a total of4859cases of yellow area, green area had a total of2830cases of patients. However, in accordance with5being the cut-off point of MEWS score,analysis showed that, MEWS score5points or less, the proportion was83.1%in the red area, suggesting that there is a big triage error rate.The four rankings with the first ranking is the most critical condition, in descending order, Statistics showed that MEWS score5points or less, the proportion was65.7%in first ranking, MEWS score5points or less, the proportion was87.1%in second ranking, suggesting that there is high triage error rate.3There had collected30patients, using MEWS and APACHE Ⅱ score showed that the correlation coefficient of the two scoring system is0.311.[Conclusion]1MEWS score has significant reference value in four rankings and three diagnostic distinction of the emergency department of one Hospital.2Using5as cut-off point of divided MEWS score, emergency hierarchical display, first caste, the misdiagnosis rate was65.7%, second caste, the misdiagnosis rate was87.1%. Emergency partitions show, the misdiagnosis rate is83.1%in the red area.Therefore, the high misclassification rate is one of the reason of the hospital congestion in emergency salvage room. Which proposal use6points as the best cut-off point in that hospital.
