Objective:To analyse the morbidity risk, the dangerous factors and prognostic factors of the patients over40years old with cerebral apoplexy in the urban area of Changde in Hunan Province in China.Methods:The professionals conduct a stroke epidemiology investigation on6095people over40years old in the central city of Changde using unified questionnaires.①The prevalence rate of this investigation is point prevalence. The calculating method is to divide the number of the new patients as well as the old ones with cerebral apoplexy by the number of the total persons who are under the investigation at the point. The prevalence rate is standarlized by the the sixth national census data standardization.②All the survey data is divided into two groups with EXCEL—cerebral stroke group and the control group. The risk factors of the disease are analyzed by Chi-square and Logistic regression.③Rankin (modified Rankin scale, mRS) scale is used to assess the prognosis of patients with cerebral apoplexy and the assessments are finally divided into two groups—good prognosis group (mRS≤2score) and poor prognosis group (mRS>2score)(The patients who have the stroke within3months will be assessed after3months), which is analysed by univariate factor and multiple factors.Results:①The investigation shows that the stroke numbers of cases in the region are628, and the prevalence rate is10.47%(10470/100000). That is8.73%according to the sixth national census data standardization (8730/100000). Among them,372are men, and the prevalence rate is12.03%(12030/100000)—9.89%(9890/100000) according to the sixth national census data standardization;256are women, and the prevalence rate is8.8%(8800/100000)—7.21%(7210/100000) according to the sixth national census data standardization. There are significantly differences in age and sex between the two groups (P<0.01).But there is no statistically difference in the educational degree, marital status and nationality (P>0.05).②The associated risk factors of the disease include hypertension, coronary heart disease, diabetes mellitus, family history of stroke, age, glycosylated hemoglobin, smoking, BMI and HDL-C.③The prognostic analysis shows that among the628cases with cerebral apoplexy,332patients have good prognosis (52.87%), and296patients have poor prognosis (47.13%). Univariate analysis shows that: there are statistically differences between the two groups in age, educational level, a history of stroke, smoking history and blood glucose (P<0.05). Multiple factors analysis shows that age, educational level, stroke type, history of stroke are poor prognosis factors for stroke patients (P<0.05).Conclusion:①The morbidity of persons aged over40years old in the urban area of Changde in Hunan Province in China is higher than that of the other regions. ②The related dangerous factors and the influencing prognostic factors for the cerebral apoplexy in Changde are similar to that in the results of other researches in China.
