The Predictive Value Of Circulating Tumor Cells (CTCs) In Ovarian Cancer: A Meta-analysis

Background Recent years, many studies have confirmed that patientswith circulating tumor cells (CTCs) in peripheral blood (PB) or bone marrow(BM) might have bad prognosis. We systematically reviewed the evidence ofpredictive value of CTCs in the circulatory system on the prognosis ofovarian carcinoma.Methods We systematically searched PUBMED, EMBASE, Cochranelibrary and CNKI (China National Knowledge Infrastructure) database tocollect relevant studies published from databases created to February2014.Quality of included studies was assessed by Newcastle-Ottawa Scale (NOS).Hazard ratio (HR) or odds ratio (OR) and its95%confidence interval(95%CI) were employed to estimated effect size with fixed-effects orrandom-effects. The available data were extracted, and meta-analysis ofthese data was conducted using stata12.0software.Results Eight studies of1184patients were included in the finalanalysis. The positive rate of CTCs was25.21%(CI,12%-60.6%). Weestimated the combined effect of overall survival (OS) and disease-freesurvival (DFS) with PB and BM group respectively. In the PB group, themeta-analysis showed that patients with CTCs-positive had significant shorter OS and DFS (HR=2.09; CI:1.13,3.88; p=0.019and HR=1.72; CI:1.32,2.25; p<0.0001, respectively). The same result of OS and DFS wasshowed in the BM group (HR=1.61; CI:1.27,2.04; p<0.0001and HR=1.44;CI:1.15,1.80; p<0.0001, respectively). We also investigated the influence ofCTCs on Figo stage, pathological Grade. But there were no significantdifference between groups.Conclusion It is obvious that patients with CTCs in their PB or BMhave shorter OS and DFS. Our research did not find exact relationshipbetween CTCs-positive cancer and later stage or poor differentiation. Moreeffective detection strategies for CTCs, high quality and large sample studiesalso need in the future.
