Clinicopathological And Prognostic Value Of Peripheral Blood Circulating Tumor Cells Before Treatment For Ovarian Cancer: A Meta-analysis

Objective: In this study,we investigated the clinicopathological and prognostic value of peripheral blood circulating tumor cells before treatment in patients with ovarian cancer.Methods: A literary search for relevant studies was performed on PubMed,Cochrane library,CNKI,Wanfang database and other databases.The language types were Chinese and English,and the time of retrieval was from the establishment of databases to February 2020.Two independent researchers screened the literature according to inclusion and exclusion criteria,extracted the data and evaluated the quality.We used the Revman5.3 software for Meta-analysis.Results: A total of 14 studies were included in this Meta-analysis.The results of Meta-analysis showed that before treatment,the difference was statistically significant in OS(HR=1.56,95%CI:1.32~1.84,P<0.00001)and PFS/DFS(HR=1.46,95%CI:1.20~1.76,P=0.0001)compared with negative group,indicating that the prognosis of patients inthe CTCs positive group was worse than that in the negative group.And also,the CTCs positive group was positively correlated with FIGO stage(OR=0.24,95%CI: 0.09~0.65,P=0.005),and the positive expression of CTCs was associated with CA125 expression and lymph node metastasis(OR=2.24,95%CI:1.35~3.71,P=0.002;OR=2.16,95%CI:1.22~3.81,P=0.008),which had no correlation with tumor cytopathological type,tumor grade and surgical method(P>0.05).Conclusion: The survival time of ovarian cancer patients with CTCs in peripheral blood before treatment is shorter than that in the negative group,suggesting that CTCs is one of the independent risk factors to determine the prognosis of ovarian cancer.It provides a new way to evaluate the prognosis of ovarian cancer patients,develop individualized treatment and clinical follow-up.
