| Objective:Colon cancer is one of common malignant tumors which is severelyharmful to human health. In recent years, the incidence and mortality rate ofcolon cancer are gradually rising, recurrence and metastasis of colon cancerare the main cause of death. Most of the pathological expressions of coloncancer are colon adenocarcinoma. Currently there is no clear colonadenocarcinoma tumor marker for the early diagnosis and treatment guidance.Osteopontin (osteopontin, OPN) is a secreted calcium has multiple functionscombined with phosphorylated glycoprotein. It is a kind of proteins related tothe tumor cell transformation, infiltration and metastasis. And with theintegrin receptor, CD44receptor binding to regulate the cell-matrixinteractions and signal transduction, enable the unrestricted proliferation ofcells to form tumors. CD44is a widely distributed transmembrane proteinexpressed on cell surface, and it is also a glycosylated transmembranepolysaccharide chain. CD44v6is a variant protein which is a differentexpression exon of CD44. It has been a long time to study on CD44and thestudy is thorough extensive. CD44is known as a widely distributedtransmembrane glycoprotein expressed on cell surface mainly involvedheterogeneous adhesion, i.e tumor cells and the host cells and host matrixadhesion, adhesion heterogeneity plays a catalytic role in the invasion andmetastasis of tumor cell. CD44v6as ligands of CD44plays an important rolein this process.Based on the study of OPN, CD44v6expression levels in normal colontissue, colon adenomas, colorectal adenocarcinoma, to study the relationshipof the OPN&CD44v6to the colon cancer occurrence and development.Providing the Molecular level foundation for the study of colon adenocarcinoma. And providing some help to the clinical diagnosis andtreatment of colon cancer.Methods:Collect80cases resection specimens of menstrual pathology confirmedcolon cancer of Baoding First Hospital from January2005to December2012,including48cases without lymph node metastasis,32cases with lymph nodemetastasis, of which47males and33females. Age35-80years old. Theaverage age is58years old. All specimens were not treated with anyanti-cancer treatment before surgery. Specimens were routinely drawn in thecenter of focus,0.5%formalin-fixed, paraffin-embedded,4μm thick serialsections. And collect20cases of normal colon tissue and30cases of colonpolyps for comparison. All specimens are adopted the immunohistochemistrystreptomycin avidin-peroxidase (SP) method to inspect OPN, CD44v6expression in the tissue sections. The results will be analyzed by SPSS17.0statistical software. The research data are count data using χ2test when acertain theoretical number1<T≤5, it must be corrected with χ2test. If P<0.05, then it will be considered as statistically significant. Using Spearman toanalyze the correlation of the two.Results:OPN, CD44v6Expression and localization observed in cells:Immunohistochemistry shows that OPN is mainly expressed in the cytoplasmof cancer cells as strong staining or moderate staining, nested or flakedistribution, some of glandular epithelial and endothelial cells are alsomoderate staining, stromal cells are no or weak staining; while CD44v6ismainly expressed on the membrane of cancer cells, and occasionally browngranules in the cytoplasm, almost no positive cells in the stroma. Positivereaction was observed by two professional pathologists who observed eachslice independently and made the judgment together. OPN is expressed in58cases of colon adenocarcinoma out of the80cases colon adenocarcinomatissues. There are7cases of positive expression out of the30cases ofcolorectal adenoma and0case of position expression out of20cases of normal colon. The positive expression rates are72.5%,23.3%and0.0%.CD44v6is expressed in64cases of colon adenocarcinoma,11cases ofpositive expression in normal colon tissues, one case expression in the colonadenoma tissue. The positive expression rates are80%,25.6%and5%. Innormal colon tissue and colon adenoma tissue, OPN expression differencesare statistically significant (χ2=5.426, P=0.02), in normal colon tissue andcolon adenocarcinoma OPN expression differences are statistically significant(χ2=34.524, P <0.001),OPN expression in colon adenoma and colonadenocarcinoma differences are statistically significant (χ2=22.818, P<0.001). In normal colon tissue and colon adenoma tissue CD44v6expressiondifference was statistically significant (χ2=6.579, P=0.01), in normal colontissue and colon adenocarcinoma CD44v6expression differences arestatistically significant (χ2=39.560, P <0.001), in the colon adenoma andcolon adenocarcinoma CD44v6expression is statistically significant (χ2=18.885, P<0.001). The differences between the expression obtained in colonadenocarcinoma, colorectal adenoma and normal colon tissues are statisticallysignificant.56cases OPN and CD44v6expression out of the80cases of colonadenocarcinoma are simultaneous positive,8cases are OPN positiveexpression while CD44v6expression is negative.8cases OPN and CD44v6expression are negative,16cases OPN expression are negative while CD44v6expression are positive. By Spearman correlation analysis, OPN and CD44v6expression in colon adenocarcinoma are positively correlated, with enhancedexpression of OPN, CD44v6expression gradually increased (P <0.05r=0.252).Conclusion:Although in the study the effects of OPN and CD44v6in tumordevelopment process is not so clear, their roles in the tumor basically clear,OPN and CD44v6may be involved in the occurrence and development ofcolon cancer. It also facilitates the Lymph node metastasis of colonadenocarcinoma. OPN and CD44v6are positive correlated with the colon cancer occurrence and development. Combined inspection the expression ofthe two in colon adenocarcinoma will help the early clinical diagnosis, degreeof malignancy, metastasis and prognosis in colorectal adenocarcinoma. |
