ObjectiveTension-type headache (TTH) is one of the most common primary headache According to epidemiological data, the global prevalence of tension-type headache is38%, half of which also suffer headache attack that can affect their daily life.Owing to the poor knowledge of pathogenesis, the modern western treatment of TTH doesn’t get satisfactory response. Besides, the intolerable side effects, poor preventive effect and cumulative toxicity of some western medicine make patients turn to traditional Chinese medicine. My mentor professor Yang Li had followed lots of authority of TCM such as Pro. Deng Tietao, Pro. Ren Jixue, Pro. Lu Zhizheng, Pro. Yao Meiling to study TCM, which made him an experienced TCM doctor. Pro. Yang cured countless Tension-type headache patients according to the theory of six meridians differentiation in clinic these years. His remarkable curative effect inspire me to sum up the experiences,establish a syndrome differentiation and treatment of diagnosis standard according to the theory of six meridians differentiation, further to statist the recent curative effect of traditional Chinese medicine medicinal in tension-type headache treatment. The study was believed to have great positive influence in promoting traditional Chinese medicine treatment of tension-type headache.MethodsThis topic takes the tension-type headache patients in the second affiliated hospital of Guangzhou university of TCM neurologist outpatient between January2007and March2014as research object, take retrospective analysis study and prospective observational study as research method, analyze epidemiological data, main symptom, medication in TCM treatment, TCM syndrome type distribution based on six meridian differentiation, the treatment effect, the effective time etc, to sum up the experiences of my mentor Pro. Yang and establish a system of six meridian differentiation treatment of tension-type headache treatment, so that it may provide certain clinical basis and new idea for further research.Results1. The epidemiological data205cases of retrospective study patients, gender ratio is as follows: the male72cases (35.1%),133cases of female (64.9%).Minimum age,13years old, maximum88years old, average46.27±16.113years old. Among them, the age less than18years old in4cases (2.0%),19-40years old79cases (38.5%),41-65years old94cases (45.9%), more than65years old28cases (13.7%).36cases of ShaoYang headache patients, sex ratio is as follows:male12cases (33.3%), female24cases (66.7%). Minimum age,22years old, maximum88years old, average age43.25±14.441years old. Among them,19-40years old patients15cases (41.7%),41-65years old20cases (55.6%),65years of age or older1case (2.8%).Among the205cases of retrospective study,160cases have no past medical history,45cases have past medical history. Among them, morbidity of sleep disorders, lacunar cerebral infarction and cerebral arteriosclerosis, menstrual most related disease, Parkinson’s disease is45.7%,20.0%,5.7%,20.0%, which shows that factors like mood, cerebrovascular, muscle tension are associated with tension-type headache.2. Six meridian differentiation syndrome type distribution205cases in retrospective study, the Taiyang headache30cases (18%), Yangming headache14cases (9%), Shaoyang headache92cases (57%), Jueyin headache6cases (4%), Shaoyin headache4cases (2%), Taiyin headache16cases (10%).Three Yang disease accounted for84%, three Yin disease accounted for12%.3. Chinese medicinal treatmentStatist percentage of TCM medicinal decoction using in six meridian headache, the results are as follows:Taiyang headache:Qianghuoshengshi decoction42.1%, Chuanxiongchatiao decoction31.6%, Guizhi jiagegen decoction31.6%;Yangming headache:Xiongzhishigao decoction38.9%, Qianghuoshengshi decoction22.2%,Dachengqi decoction16.7%;Shaoyang headache:Yetianshi decoction64.0%%,Xiaochaihu decoction:28.9%;Taiyin headache:Yiqicongming decoction and Qingzhendecoction:56.5%, Banxiabaishutianma decoction:34.8%;Shaoyin headache:Mahuangfuzixixin decoction:50%, Huanglianejiao decoction:50%;Jue Yin headache:Tianmagouteng decoction:30%, Zhenganxifeng decoction:20%, Wumeiwan decoction:20%, Wuzhuyu decoction:10%,Dangguisini decoction:10%, Baitouweng decoction:10%. In prospective observational study,36cases of shaoyang disease patients with headache,15cases (41.7%) use Xioachaihu decoction,17cases (47.2%) use Ye Tianshi decoction,4cases (4%)use Longdanxiegan decoction, the result is similar with retrospective study.4. Accompany symptomsprospective observational headache cases,36cases of shaoyang disease with bitter mouth and dry throat31.1%, chest tightness58.3%, nausea44.4%, vomit22.2%, alternate chills and fever0%, poor appetite2.3%, poor sleep8.3%.5. The curative effect analysisAfter1month of therapy,the curative effect analysis of205cases in retrospective study is as follow:72cases cure, accounted for35.1%;122cases better, accounting for59.5%;11cases not healed, accounting for5.4%.36cases of shaoyang disease headache prospective observational case, by t testing before and after the two visits in SF-MPQ rating-score, emoti onal score, sensory score comparisons of were statistically significant d ifferences. According to SF-MPQ scale to calculate the total score eval uation of curative effect, after treatment is4cases effective, account ing for11.1%;31cases were markedly improved (86.1%);1case recovered accounted for2.8%. Conclusion1. This study sex ratio, age distribution, factors related to medical history and literature basic consistent epidemiological data;2. Tension-type headache six-channel differentiation three Yang disease, syndrome type distribution of three Yin disease;3. On the basis of on the fusion of science of seasonal febrile diseases, referring the experience of the past dynasties doctor, formed by six-channel syndrome differentiation system is proved effective.
