Objective: To compare the clinical effect of video-assisted mini-thoractomythoracotomy, single utility port hybrid video-assisted thoracoscopic lobectomy andcomplete thoracoscopic lobectomy,and explore the clinical value of single utility porthybrid video-assisted thoracoscopic lobectomy with patients of early stage Ⅰ lungcancer.Methods:Eighty patients with early-stage non-small cell lung cancer underwentthoracoscopic lobectomy from Oct.2008to Dec.2012.Video-assisted mini-thoractomythoracotomy was conducted in26patients(VAMT group),single utility port hybridvideo-assisted thoracoscopic lobectomy was conducted in30patients(hybrid group),and24patients received complete thoracoscopic lobectomy(complete thoracoscopicgroup).All patients were identified clinical stageⅠbefore operation,and the general datawere similar among the3groups. The number of sampled or dissected lymph nodes,operation duration,intraoperative blood loss,the time for postoperative catheterization,postoperative hospital stay,incidence of postoperative complications,the scores ofpains dictated by the patients on the5th day after the operation and hospitalizationexpenses of the three groups were compared.Results: There were no statistical differences in all the indexes mentionedabove(p>0.05)except the operation time and the hospitalization expenses between thehybrid group and c-VATS group.The operation time of hybrid group was longer thanthat of c-VATS group,and the expense of hybrid group was less (p<0.05). There were no statistical differences in the number of sampled or dissected lymph nodes in VAMTgroup compared with the other two groups(p>0.05),but there was statistical differencesin all the other indexes(p<0.05).Conclusion: Single-operation-port hybrid thoracoscopic pulmonary lobectomyis as minimally invasive as complete thoracoscopic pulmonary lobectomy,but betterthan VAMT in minimal invasiveness. Single-operation-port hybrid thoracoscopicpulmonary lobectomy is easier to master relatively and can reduce the hospitalizationexpenses,so it is worthy of promotion in primary hospitals.
