Objective:The aim was to identify the clinical factors and tumor characteristics th-at improve our knowledge of the diagnosis and treatment in patients older than75yearswith colorectal cancer.Methods:268cases of patients over75years of age with colorectal cancers in FirstAffiliated Hospital of Dalian Medical University from2003to2012.All of them hadreceived surgery treatment and pathologically confirmed. Retrospectively analyze thepatients clinical data include:age,gender,relevant auxiliary examination,surgical approa-ch,postoperative pathology,complication and so on.Their clinical variables were analyz-ed by χ2test or Fisher test.Results:268cases of patients accounted for16.04%of the patients with colorectalcancer over the same period.There were no significant difference of gender between theage groups.Clinical manifestation in the268older patients was mainly performed ashematochezia/mucous bloody stools(53.36%).There were difference of clinicalmanifestation between the age groups.The incidence rate of hematochezia/mucousbloody stools and abdominal pain are significantly higher in the young group than in theold group,while the incidence rate of abdominal mass in the young group issignificantly lower than in the old group.There were no significant difference of diseasesite between the age groups.Gender was difference of disease site between the age groups(P<0.05).TNM stage,tumor size,macroscopic type were no significant differenceof age groups.Every age groups are mainly in Ⅱ stage of TNM stage (44.78%), tumorsize are mainly in2-5cm(55.60%),macroscopic type are mainly in ulcerative(59.33%).Histological grade was difference between the age groups(P<0.05),the incidence rate ofmiddle differenriated in the young group was significantly higher than the old group,thebetter differenriated in the old group was significantly higher than the young group.Ly-mph node metastasis,complication,surgical approach were no significant difference ofage groups.We found that elderly patients with colorectal cancer had more non-specificcomplications,The higher incidence rate of complications was incisions infection(8.96%),secondary was anastomotic leakage and bleeding,posto-perative mortality being assoc-iated with the presence of postoperative complications.Conclusion:1.Elderly patients with colorectal cancer symptoms are not typical,early diagnosis is low,and has a better histological differentiation,Lymph node metastas-is is low,Preoperative complications and postoperative complications relatively moreoccurred.2.Age is not a major factor limiting surgery.These results suggest that surgery isfeasible and can be safe for older patients, but it needs much more evaluation ofcomorbidities,pre-and post-operative intensive care to avoid post-operative non-specificcomplications,that improve survival and quality of life of elderly patients.
