Objective:Through the research on the risk of cerebral infarction accompanied by white matterlesions of the patients, to make clear that whether there are promoting factors will increase theincidence of acute cerebral infarction and lacunar infarction with the white matter lesions, andto investigate whether promoting risk factors make the resting cerebral lacunar infarction acutecerebral infarction increased,which may found some reference for the prevention and cure ofthe cerebral vascular disease.Methods: The sample included252cases of patients with ischemic cerebral vascular diseasetreated in internal Medicine-Neurology of our hospital from2012February to2013December.On the basis of magnetic resonance imaging (MRI+DWI),the cases were dividedinto five groups,cerebral white matter lesions (WML,125cases), acute cerebral infarction groupassociated with white matter lesion (aCI+WML,32cases,), silent lacunar infarction group withmatter lesions (LI+WML,43cases), lacunar infarction group (LI,28cases), acute cerebralinfarction group with silent lacunar infarction (aCI+LI,24cases),also make clear the knowledgeof the patient’s age, gender, smoking, drinking, diabetes, hypertension, H type-hypertension,cholesterol, low density lipoprotein, high density lipoprotein, glycerin three fat, allof these factors were compared between groups in WML group and aCI+WML group, WMLgroup and LI+WML group, LI group and aCI+LI group by chi square test and Logisticregression model, analysis of risk factors on the influence of cerebral infarction situation withcerebral white matter lesions and lacunar infarction.Except theWML+LI cases, the cases weredivided into acute cerebral infarction (aCI,56case) and non acute cerebral infarction(nCI,153cases), analysis the distribution of cerebral white matter lesion and lacunar infarctionin two groups by analysis of chi square test Condition; all the data were analyzed by SPSS17.0software, with P <0.05difference has statistics significance.Results:1.By chi square test between WML and group aCI+WMLfound that age, gender, hypertension,H type-hypertension, cholesterol,had statistical significance in the difference between the twogroups (P <0.05), by Logistic regression model analysis found, cholesterol, low densitylipoprotein with statistical significance in the difference between the two groups (P <0.05);cholesterol increased risk of cerebral white matter lesions of acute infarction more than12times (OR=12.677); elevated low density lipoprotein so that the risk of cerebral white matterlesions in acute infarction increased more than3times (OR=3.459); 2. By chi square test between WML and group LI+WML found that, hypertension, elevatedcholesterol in the difference between the two groups was statistically significant (P <0.05);model analysis for Logistic regression found, hypertension, elevated cholesterol in thedifference between the two groups was statistically significant (P <0.05), cholesterol made therisk of patients with white matter lesions lacunar infarction increased more than18times(OR=18.219), the risk of hypertension cerebral white matter lesions silent lacunar infarctionincreased more than7times (OR=7.120);3. By chi square test and Logistic regression model analysis of LI and group aCI+LI found thatthe factors of the difference between the two groups were not statistically significant (P>0.05);4. Silent lacunar infarction compared to the cerebral white matter lesions group, the occurrenceof acute cerebral infarction with statistical significance in the difference between the twogroups (P <0.05), lacunar cerebral infarction risk during acute cerebral infarction is larger,about3.348times the white matter lesions (OR=3.348).Conclusion:Cholesterol is an independent risk factor of lacunar infarction and acute stroke forpatients with cerebral white matter lesions; low density lipoprotein is an independent risk factorof acute stroke in patients with white matter lesions; Hypertension is an independent risk factorfor cerebral white matter lesions lacunar infarction;The people with resting lacunar in farction have a higher risk of acute infarction comparedwith cerebral white matter lesions.
