Objective:The aim of this paper was to study the level changes of serum YKL-40(chitinase-3-like protein1) in patients with chronic heart failure(CHF) and factors that affect short-term prognosis in CHF. Methods:A total of120CHF patients hospitalized from June to October2011in the department of cardiology of the Hunan Provincial People’s Hospital, including68patients of ischemic heart failure and52patients of non-ischemic heart failure.There was the normal control(NC) group of50patients. serum of YKL-40levels were measured by ELISA method on day1and day7after admission.All patients were followed up for3months.Readmission or death was the end point.Results:The serum YKL-40levels in patients with CHF were higher than those patients in the NC group (P=0.03).Serum YKL-40levels were not statistically significant(P=0.48) between ischemic CHF and non-ischemic CHF patients.Serum YKL-40level in Ⅱ class patients of CHF with New York Heart Association(NYHA) functional class is lower than those patients in the Ⅲ, Ⅳ class CHF groups (P=0.048, P=0.01).Serum YKL-40levels were not statistically different (P=0.995) between Ⅲ and Ⅳ class CHF patients. Serum YKL-40level of patients with CHF treatment was significantly lower (P=0.04) than those patients before treatment. According to the median of YKL-40level, patients with CHF divided into two groups (low concentration group≤92.97, high concentrations of>92.97).Poor prognosis of high concentration group were significantly higher than the low concentration group (χ2=6.998, P=0.010). Impact of CHF short-term prognosis risk factors using logistic regression analysis,Serum YKL-40levels and CHF-term prognosis were related(P=0.010). Conclusion:1, Serum YKL-40levels with CHF patients were higher than those patients of the NC group;2,Serum YKL-40levels in Ⅲ, Ⅳ class of CHF patients with NYHA functional class were higher than those Ⅱ-class patients of NYHA class;3,Serum YKL-40levels in patients after treatmen with CHF were decreased;4, Serum YKL-40levels is an independent risk factor for a short-term prognosis in patients with CHF.
