Objective: To investigate the relationship between postoperative joint pain and theusing of bone cement in hip arthroplasty.Methods: From January2007to February2012, there are40patients withtranscervical fracture and48patients with ischemic osteonecrosis of the femoralhead.50patients were treated with the bone cement hip replacement, and38patientswere treated with the cementless hip replacement. To Respectively compare the twomethods of treatment in postoperative3months,6months,1year,2years about hippain, score according to the Harris and the subentry percentage hip pain. The patientsof bone cement hip replacement in postoperative2years were divided intonon-loosening group,X-Ray loosening group and clinical loosening group accordingto radio graph for analysis of the hip pain after THR. We did the statistics analysis bySPSS17.0software. P<0.05has statistic significance.Results: The88cases had complete follow-up records and a mean follow-up of28.5months(range,24to48months).1. Score according to the Harris and the subentry percentage hip pain. There issignificant difference in the incidence of the hip pain between the bone cementhip replacement group and the cementless hip replacement group (P<0.05)after postoperative3months,6months,the bone cement hip replacement group isbetter than the cementless hip replacement group. There is significant difference inthe incidence of the two groups(P<0.05)after postoperative1year,2years,thecementless hip replacement group is better than the bone cement hip replacementgroup. In the cementless hip replacement group,there is not significant difference inthe incidence of postoperative3months and6months(P>0.05), and there issignificant difference in the incidence of postoperative1year and2years(P<0.05),the pain gradually ease over time. In the bone cement hip replacement group,there is not significant difference in the incidence of postoperative3monthsand6months(P>0.05), and there is significant difference in the incidence ofpostoperative1year and2years(P<0.05),the pain gradually aggravated over time.2. There is not significant difference in the incidence of the hip pain betweenX-Ray loosening group and non-loosening group(P>0.05),but there is significantdifference between clinical loosening group and non-loosening group, X-Rayloosening group(P>0.05).Degree of the hip pain is in correlation with the degree ofthe prosthetic loosening.Conclusions:1. The two methods of treatment are obvious effect of pain relief. Score in pain:the bone cement hip replacement group is better than the cementless hip replacementgroup in6months. But, the cementless hip replacement group is better than the bonecement hip replacement group after1year.2. The incidence of hip pain increased after1year about the bone cementhip replacement group.The prosthetic loosening is the main cause of the hip pain aftertotal hip replacement. There are positive connection between the degree of hippain and the degree of prosthetic loosening.
