Objective:To evaluate if converse Z epicanthoplasty has any notable aesthetic impact onpatients compared with transverse-vertical plasty.Method:Two hundred and two consecutive patients underwent epicanthoplastywere enrolled in this prospective randomized pilot study from February2012to July2013. According to the severity of epicanthus, the patients were divided into threegroups: Group A (mild),Group B (moderate) and Group C (severe). Then, Group Acan be randomly dividied into group a1which was using the converse Zepicanthoplasty and group a2which the transverse-vertical plasty is adopted.Similarly, the Group B was divided into group b1and group b2; Group C into groupc1and group c2. three months later,the scar aesthetics were evaluated by patients andsurgeons using the Patient and Observer Scar Assessment Scale (POSAS).Meanwhile, digital photographs about scars were taken and assessed by non-researchrelated viewers.Results:There were198cases who received scar aesthetic assessment in all groups.There were not any comparative demographic characteristics between Group A,Group B and Group C. The overall patients’ satisfaction between group a1andgroup a2has no statistical significance(P=0.241). Group a1got (2.10±0.20)scores and group a2got(2.20±0.32)scores respectively. The overall patients’satisfaction between group b1and group b2has statistical significance(P=0.010).Group b1got(2.44±0.47)scores and group b2go(t4.55±0.55)scores respectively.The overall patients’ satisfaction between group c1and group c2has statisticalsignificanc(eP=0.041).Group c1got(4.70±0.71)scores and group c2go(t6.00±0.53)scores respectively. There were significant differences in scar assessment scale scorefor other scar assessment scales(including PSAS score and OSAS score)betweengroup b1and the group b2, and between group c1and group c2. The evaluation of digital photographs about scars by non-research related viewers was significantlydifferent as well(P<0.05).Conclusion:From the limited cases of this experiment, we can draw a conclusion. For mildepicanthus, the patients’ satisfaction for postoperative scar between two methodshave no differences; But for moderate and severe epicanthus, the patients’satisfactionof converse Z-epicanthoplasty is higher than transverse-vertical plasty.
