Background: The formation of scars after orthopaedic neoplasia is a common clinical phenomenon at present.Although the scars will gradually fade around 6 months after the operation,the formation of scars in the early postoperative period not only affects the appearance,but also affects the shape of the double eyelid.The problem troubled plastic surgeon for a long time.It was reported that botulinum toxin A(BTXA)can prevent the formation and development of scars.However,there are few reports on the application of scar prevention after eye surgery at home and abroad.Objective: To explore the effectiveness and safety of botulinum toxin type A combined with Coractubex in preventing scars after epicanthoplasty.Methods:We Randomly selected 40 patients who underwent a “epicanthoplasty by transverse incision+ dualeyelidoperation by incision ”at the First Affiliated Hospital of xxx Medical University from May 2018 to May 2019.The operation was completed by the same doctor,and randomly divided into the experimental group(20 cases)and the control group(20 cases)according to the different postoperative intervention methods.The experimental group was injected with 0.2cm subcutaneous injection of the new medial point away from the edge of the eyelid immediately after surgery.Type 2U of botulinum toxin type A,the midpoint of the "V" type incision line was 0.2 cm subcutaneously from the marginal side of the eyelid.Type 2U of botulinum toxin type A was injected subcutaneously.The experimental group and the control group were treated with Contractubex 10 days after surgery Apply a thin layer of Contractubex on the surgical incision area,massage gently until it is completely absorbed,three times a day,and use it until 6 months after surgery.The patients need to review at 3 months and6 months after surgery.Two experienced plastic surgeons uniformly made an evaluation,the left eye of each patient was used with scar width,modified Vancouverscar scale(VSS)and Visual Analog Scale(VAS),At the same time,we also did an independent evaluation of patient's satisfaction rate,we evaluated the scar conditions at the inner canthus 3 and 6 months after operation and then did statistical analysis.Results: There are no statistically significant differences in the male to female ratio between the two groups(experimental group: female 19 male 1,control group:female 20 male 0),age difference(experimental group: 26.5,control group: 26.2).No complication related to the use of botulinum toxins such as tears,drooping of the upper eyelid,and eversion of the eyelids occurred.There are no complications such as poor wound healing and infection.At 3 months after operation,compare the experiment group with the control group about the scores of scar width,various sub-items(color,thickness,vascular distribution,and softness)of modified VSS and The differences were significantly.(P <0.05),and the satisfaction rate of patients in the experimental group was higher than that in the control group;at 6 months after operation,the scar width and the thickness in the modified VSS scar scale were significantly different from those in the control group(P <0.05),the color,blood vessel distribution and softness,and the VAS scale score in the modified VSS scar scale were not statistically significant compared with the control group(P> 0.05),and the satisfaction rate of the patients was higher than the control group;Statistical analysis was performed at different times of 3 months and 6 months.It was found that the width,color,thickness,vascular distribution,softness,pain,and itching of scars at 3 months and 6 months both have statistical significance.(P <0.05).Conclusions: 1.Botulinum toxin Type A(BTX-A)combined with Contractubex can prevent the formation and development of scars after epicanthoplasty,the pain and itching caused by postoperative inflammation and scar hyperplasia,It is worth promoting in clinic.2.Botulinum toxin type A(BTX-A)combined with Contractubex is more effective than Contractubex alone in preventing scar growth after epicanthoplasty,which can achieve a higher patients' satisfaction rate.
