Objective To investigate factors associate with small chylous leakageafter laparoscopic radical gastrectomy in gastric cancer.Methods Clinical and pathological data of1366patients with gastriccancer undergonging laparoscopic radical gastrectomy in gastric cancerfrom2009January to2012in our department were analyzed retrospectively,postoperative chylous leakage clinical and pathological datas of with(postoperative chylous leakage group)and without small patients withchylous leakage (no chylous fistula group) were analyzed retrospectively.To explore the relationships between the small chylous fistula andpostoperative metastasis of gastric lymph node,risk factors assoctiatewith postoperative chylous leakage were investigated by univariate andmultivariate analysis.The recovery were also analyzed between twogroups.Results All of the1366cases of chylous leakage,57cases (4.17%), nochylous leakage were1309cases (95.83%).By logistic regressionanalysis,preoperative anemia and hypoalbuminemia, number of lymph nodedissection, tumor N staging and metastasis in satation No.7or No.8aresignificant factors (P <0.05). Multivariate analysis revealed that, thenumber of lymph node dissection>35、tumor pN3and metastasis in satationNo.7or No.8were independent risk factors of chylous leakage afterlaparoscopic radical resection of gastric cancer (P <0.05). Compared withthe no chyle leakage group, chylous fistula patients postoperativehospitalization time was significantly prolonged (P <0.05).Conclusions Lymph node dissection number≥32, the number of lymph nodemetastasis≥3, N0.7, NO.8group of lymph node metastasis in patientswith laparoscopic radical gastrectomy for gastric cancer after small chyle leakage significantly increased risk.
