Objective:Analyze the influences on the dose of glucocorticoid in the treatment of severe drug eruption.Methods:All ceses were the inpatient of The First Affiliated Hospital of Fujian Medical University、The First Affiliated Hospital of Xiamen University and Quanzhou First Hospital during2010.1to2012.12.The diagnoses fo all cases corresponding to the diagnosis standards of severe drug eruption.We used effective control-dose of glucocorticoid and cumulative effective dose of glucocorticoid as dependent variables、investigated sex、age、drug eruption type、lesion atra、epidermolysis area and SCORTEN Score as independent variables,and statistics by single factor analysis、correlation analysis and multiple stepwise regression analysis respectively.Results:The major drug eruption type in this study is SJS(41.9%),TEN next(34.9%),and DRESS least(23.2%),and male:femail=1.26:1.Compared with SJS and TEN groups, there is a decrease in the age in DRESS group.The major allergenic drug is antiuepileptic drug(30.2%),antibiotics second(27.9%),and antigout drug third (14.0%).Single factor analysis showed that the control-dose of glucocorticoid in different drug eruption type groups and different epidermolysis area groups had a statistical significance(P<0.05),while the cumulative effective dose of glucocorticoid in different drug eruption type groups and different epidermolysis area groups had not a statistical significance(P<0.05).Correlation analysis showed that,there may be correlation among drug eruption type、lesion atra、epidermolysis area and the control-dose of glucocorticoid;there may be correlation among drug eruption type、epidermolysis area and the cumulative effective dose of glucocorticoid. Multiple stepwise regression analysis showed that drug eruption typeand epidermolysis area may have influences in control-dose of glucocorticoid and cumulative effective dose of glucocorticoid. Conclusion:1、The major drug eruption type in this study is SJS,TEN next,and DRESS least.Male subjects are more possible to occur severe drug eruption.Compared with SJS and TEN groups,there is a decrease in the age in DRESS group.The major allergenic drug is antiuepileptic drug,antibiotics next.2、Drug eruption type、lesion atra and epidermolysis area should be considerde when choosing the dose of glucocorticoid in clinic,and increase the dose appropriately to control the disease as soon as possible.
