Objective1.To analysis the correlation between the increased levels of uricacid and peripheral neuropathy.2. To evaluate the relationship about the increasedlevels of uric acid and insulin resistance in type2diabetes mellitus. Methods Inthe patients with type2diabetes mellitus, the motor conduction velocity of thenon-dominant side limbs nerve which including median nerve,ulnar nerve,tibialnerve,peroneal nerve were detected, while,the sensory conduction velocity alsowere measured which including median nerve,ulnar nerve,sural nerve.Based on theresults of the motor, sensory conduction velocity, peripheral neuropathy isdiagnosed,if one of them is abnormol.120type2diabetes mellitus with peripheralneuropathy were divided into two groups.One group which with high level of uricacid including34cases, while86cases were included in another group whichwith the normal level of uric acid.In each group,gender, age and duration of allthe patients were recorded.At the same time, microalbuminuria(MA),fasting bloodglucose determination (FBG),fasting c-peptide, glycated hemoglobin (HbA1c),theTC (total cholesterol), TG (triglycerides), high-density lipoprotein cholesterol(HDL-C), low density lipoprotein cholesterol (LDL-C),body mass index (BMI)and insulin resistance index (HOMA-IR)(CP) were calculated.Results1.TheMA,TG, fasting serum c-peptide, homa-ir index (HOMA-IR)(CP) were higher inthe HUA group than that in another group.[(103±10) mg/L,(75±15) mg/L; (5.4±2.1) mmol/L,(4.7±1.0) mmol/L;(9.4±4.3) ng/ml,(7.3±3.0) ng/ml;(1.3±0.22),(1.1±0.31);P <0.05];The sural nerve conduction velocity in the HUAgroup is lower than that in another group [(43.4±4.6) m/s,(49.5±6.6) m/s, P <0.05).2.Spearman related analysis results show that uric acid was positivelyrelated with MA, TG, fasting serum c-peptide, homa-ir(CP)(HOMA-IR)(CP), BMI(r=0.211、0.267、0.320、0.279、0.317, P <0.05),while,the uric acid was negativelyrelated with sensory nerve conduction velocity of sural (r=-0.265, P <0.05);Adjusted for age, duration, obesity and insulin resistance factors, the partialcorrelation analysis shows that, the uric acid was still negatively related withsensory nerve conduction velocity of sural (r=-0.240, P <0.05). Conclusion1.The increased levels of uric acid were closely related with type2diabeticperipheral neuropathy.2. The uric acid was positively correlated with insulinresistance.3. Comparied with the upper limbs, the damage of the lower limb nervesespecially the sensory nerves were involvemented earlier and the symptoms weremore serious.
