ObjectivesTo discuss the function and significance of urinary endothelin-1(ET-1) in the diagnosisand long-term follow-up of children who suffer from unilateral ureteropelvic junctionobstruction.MethodsBased on the randomized approach,the study included30children with unilateralureteropelvic junction obstruction who underwent pyeloureteroplasty in guangzhouwomen and children’s hospital.30cases of control group children, including10cases ofhealthy children,10cases with renal calculi,and10cases with vesicoureteral reflux.In thestudy group, urine specimens were obtained from the children before and duringpyeloureteroplasty.These children also offered bladder urine specimens in1,3,6,9and12months after surgery. The30control group children also preserve urine specimens.In allurine specimens, ET-1value was tested by enzyme-linked immuno sorbent assay, at thesame time urine creatinine was measured.ResultsUrinary ET-1level of the study groups before surgery was significantly higher than theother three control groups, the difference was statistically significant (P <0.05).In the30 children who underwent pyeloureteroplasty,ET-1had descended significantly at12months postoperatively.In all study samples,a diagnostic boundary value of3fmol/mg Crgave a sensitivity of76.7%, a specificity of90%, and an overall accuracy of83.3%.In theeight infants (younger than1year old) samples, used5fmol/mg Cr as diagnosticboundaryor value, the sensitivity and specificity were100%for both.ConclusionsThe urine ET-1can be a effective index for the diagnosis of children with ureteropelvicjunction obstruction. As a biological marker,urinary ET-1is noninvasive,convenient andspecific.It is useful in the diagnosis and the long-term follow-up for children with UPJO.
