As a part of the basic research about the Shanghai Special Equipment Emergency Management platform construction, the paper’s main work is to research the risk of the cylinder inspection agency. As one kind of pressure special equipment, cylinder has the characteristics of large amounts and high accident rate. As a very important aspect, the periodic inspection has been conducted to ensure the safety in the use process of cylinders and decrease the accident rate. Considering the inspecting quality of the cylinder inspection agency and the uneven safety condition, it is necessary to conduct a comprehensive risk research on the cylinder inspection agency. In this paper, a series of steps, including identifying risk factors, determining the index system, building the risk evaluation model, have been conducted to eventually establish a set of risk research system for cylinder inspection agency. The main contents are summarized as follows:(1) Cleared that the risk of cylinder inspection agency includes not only the inspection security risk, but also the inspection quality risk. The risk factors of the entire inspection process were identified in detail from these two aspects and analyzed the deep reasons with the accident-causing theory on the basis of risk identification and finally proposed the accident causing double route model of the cylinder inspection agency.(2) Established the index system of both the organization and human with the results of accident-causing analysis, the organization index system was established based on the evaluation criteria of the safety management system and requirements of the special equipment inspection and testing institutions quality management system, the human index system was established based on the unsafe behavior theory and the human error theory.(3) The cylinder inspection organization risk evaluation model was established. Using the combination weighting approach based on the decision maker’s preference and consistency of weighting methods to combine the weight of AHP and entropy to evaluate the organization index. Design the assessment questionnaire to evaluate the human index. The final risk grade was concluded by the amendment to organization grade with human grade.(4) The case of liquefied petroleum gas inspection agency was assessed in this paper, the results were consistent with the actual situation and verified the practicability and operability of this model.
