Purpose:This research has2purposes. The first one is to design a dysarthria rehabilitation system with the technology of computer-aid. So this system can be a good helper of the clinical therapists. The second purpose is to do a preliminary trial to test the clinical effect of the system.Method:1. The design of the dysarthria rehabilitation system.On the basis of preclinical rehabilitation programs of dysarthria in clinical, we selected some items that can be trainsformed into computer modules to develop a primary project, according to the characteristics of computer-assisted training. A group that contained a senior rehabilitation physician and3long been engaged and proficient dysarthria rehabilitation physicians discussed the project repeatedly to form the final project.Two software engineers that had experience in the production of rehabilitation programs trainsformed the final project into a dysarthria rehabilitation system. The software engineers debug the system according to the feedback from the therapists and the patients in the clinical trial.2. Observation of the clinical effect of Computer-aided dysarthria rehabilitation system. After the system is completed, we used the randomized controlled trial to test it. Based on inclusion and exclusion criteria,60cases of post-stroke patients with dysarthria, were divided into experimental group and control group (30cases),using a random number table. Artificial training was used in control group while the artificial training combines with the computer-aided training dysarthria system was used in test group. Intervention time is3weeks,5times a week, every30min.The modified Frencchay dysarthria rating scale was used to assess before and after treatment to measure the clinical efficacy.Results:1. The study completed the dysarthria training system.Dysarthria training system is divided into articulators sports training and voice training two parts, a total of14small modules. Articulators training section includes the head and neck relaxation training, vocal relaxation training, breathing exercises, jaw movement training, lip exercises, tongue training, velopha-ryngeal training modules. Voice training includes prolonged pronunciation training, volume training, speaking speed training, tone training, voice reading training, rhythm training, tongue twisters training.2.60quaified patients with dysarthria after brain injury were collected.57patients completed the trial, shedding3cases. The modified Frenchay dysarthria rating scale was used to assesse the condition of the dysarthria,The condition can be divied into normal, mild impairment, moderate impairment, severe impairment, very severe impairment according to the scores of the scale. After statistical analysis, the control group after treatment compared with before treatment there was a significant difference (P<0.05), there was a significant difference (P<0.05) after treatment compared with before in the test group. There was no significant difference (P>0.05) before treatment experimental group and the control group. There was a significant difference (P <0.05) after the test group and the control group dysarthria assessment level. Markedly effective rate and the experimental group had slightly higher efficiency than the control group.Conclusion:This research successfully built a dysarthria rehabilitation system based on computer-aid technology. The system contains abundant modules and the contents are scientific and comprehensive. It is useful to treating the dysarthria after brain injury.
