Background: With society and economy developing, and medical health system deepreforming, people demand medical services for higher and higher requirements,therefore the present modes of medical education and personnel training apparently lagbehind the demands of medical health structure reform and requirements of thehigh-level medical services. Chinese current medical education mode centrals majorlyon disciplines, and curriculum of each discipline is relatively independent, is dividedinto too many branches, resulting in overlapping and repetition. The teaching methodsare mostly to cram, and verifying experiments pervade the experiment teaching thatbelongs to theoretical teaching; Curriculum evaluation mainly focus on memorization,and thus it presents the following problems: the students having classes extra time andoverburden, paying inadequate attention to ability culturing of independent thinking,problem analyzing and solving. For this reason, the Ministry of Education and theMinistry of Health held a joint conference of the national medical education reformproviding an overall design and systematic arrangements to boost medical educationreform at the end of2011. The Ministry of Education and the Ministry of Health jointlyissued an announcement initiating ‘the Outstanding Doctor Training Plan’ to optimizetraining structure of clinical talents in May,2012. In view of this, we carry out thematicresearch on personnel training mode reform in clinical medicine.Objectives: Through studying on evolution history and current situation of five-yearclinical medical education, analyzing shortages of original mode of knowledgeaccumulation-oriented talent cultivation, we established a theoretical cultivation modeof Outstanding doctors majoring in five-year program of clinical medicine complying by medical mode transition and reform requirements of health service system, basing onmedical educational accrediting standards, led by medical talents competence, targetingon ‘Strengthen humanity, Consolidate foundation, Improve skills, and Enhance quality’.Method: Analyzed the problems existing in cultivation mode of domestic Outstandingdoctors majoring in five-year program of clinical medicine and compared currentsituation and features of training patterns between domestic and abroad; Clarified theemphasis and content of program and protocol the framework and innovation strategiesof Outstanding doctor cultivation mode by expert consultation and seminars; Organizedand analyzed the collected data by data analysis method in order to complete theprotocol and draft a theoretical model of cultivation mode for Outstanding doctorsmajoring in five-year program of clinical medicine.Results:1. We identified the goals of Outstanding doctor training: deepen educationreform,readapt to medical reform, qualify positions, prioritize ability training’, as aprinciple, and focus on consolidating medical students humanistic traits and clinicalpractical ability, to set up the goals for Outstanding doctor training.2. We established an Outstanding doctor training scheme: formulating talent trainingscheme guided by medical students’ competency. Students should be educated to careabout patients and respect lives as their occupational ethics through strengtheningeducation on humanism and professional moral to intensify their professional moral.We integrated courses and optimized course system and teaching program to transmit‘teaching system divided by subject” to ‘basic integrated courses centered by systemand clinical integrated courses guided by diseases’. The practice teaching system shouldbe enhanced, and clinical practice teaching should be strengthened to achieve “early,much and reduplicative clinical practice”, and train students on problem-solving abilities. Teaching reform centered by students should be promoted to reform teachingand assessment methods, and develop assessment system combined formation tofinality.3. Improved quality guarantee measures: updating ideas of teaching management andestablishing a teaching quality guarantee system which comprises one goal (medicaleducation approval standards), two holders (theory and practice teaching), and threeplatforms (theory teaching in teaching staff room, experiment teaching in laboratory,and practice teaching in clinical departments). We implemented the teaching personnelconstruction policy as ‘apt structure, better traits, appropriate quantity, vigorous, andstable’ to enhance teaching staff construction, and consolidated hardware equipmentconstruction and guarantee practical ability cultivation, to make clinical medicinebecome special major with distinctive characteristics, good style of study, leading in ourprovince, advanced in China, and play demonstrative role in provincial medical collegesin mid-western China.Conclusions: Redesigned and formulated the cultivation scheme of Outstanding doctorsmajoring in five-year program of clinical medicine; Proposed to construct theoreticalframework and reformative measures of ‘strengthening humanism, consolidatingfoundation, emphasizing on skills and focusing on traits’ due to the problems existing inthe present educational mode, such as weak humanism education, irrational curriculumstructure, and lack of practice teaching. The whole humanism education andcomprehensive practice teaching system facilitate cultivating clinical talents who arecompetent, highly qualified, and desired by people.
