| BackgroundHemodialysis is one of the key measures to treat the end-stage renal disease patients (ESRD). But hemodialysis complications have been greatly reduced and the quality of life has been improved with the diagnosis and treatment level improvement of kidney disease and the rapid development of the alternative treatment technologies in recent years. But even so, long-term patients with uremia hemodialysis face all sorts of complications, thirst is one of common complications. But dialysis patients are mostly oliguria or anuria, if too much water intake and fail to dialysis excess water in the body, which will cause the overload capacity, increase the burden of cardiovascular (for example high blood pressure、edema、heart failure and so on)、affect the dialysis adequacy、worsen kidney function further, eventually increase patient’s risk of heart, cerebrovascular complications and death. Therefore control intake and thirsty became a pair of sharp contradictions in front of dialysis patients.So we urgently need a simple and effective method to improve the symptoms of patients with thirst, control weight gain during the period between dialysis.So as to reduce the heart and cerebrovascular complications.The ear pills therapy [1-2] joint chewed medlar [3] is a kind of curative effect of treatment method. Because which not only can obviously improve symptoms of thirst in maintenance hemodialysis (MHD) patients, but also can significantly reduce the weight gain.ObjectiveWe regard ear pills joint chew medlar as one of the overall treatment methods of hemodialysis patients with thirst. So as to improve the thirsty symptom of hemodialysis patients, control the weight gain in dialysis interphase,further reduce the cardiovascular complications and improve patients’ quality of life. At the same time, giving full play to the advantages of traditional Chinese medicine treatment and provide a certain basis for the treatment of uremia patients’ thirsty.MethodThis study selected50MHD patients with thirsty and gived treatment of ear pills in combination with chewed medlar on the basis of conventional treatment for8weeks. The main acupuncture point is according to the points chosen principle and the thirsty pathogenesis of Chinese and western. Therapeutic effect index is based on rating scale to assess degree of thirst and at the same time monitoring the weight gain between dialysis period, Kt/V, serum sodium concentration, blood pressure etc. Comparing the indexes’ changes before and after treatment. At the same time explore the correlation of thirsty to weight gain between dialysis period、KT/V value、mean arterial pressure before dialysis, serum sodium concentration before dialysis、age etc.Results(1)Thirsty degree score of50cases was decreased significantly after4and8weeks,which compared with before treatment (P<0.01),and Thirsty degree score of50cases was decreased significantly after8weeks compared with4weeks (P<0.01).(2)The average weight gain relative treatment between dialysis period was also decreased significantly after4and8weeks compared with before treatment (P<0.01),and decreased significantly after8weeks compared with4weeks (P<0.01).(3) Kt/V value and serum sodium concentration were no statistically significant difference before treatment respectively compared with4and8weeks,4weeks compared with8weeks (P>0.05).(4)Thirsty rate and the average weight gain between dialysis period were positively correlated (P<0.01).(5)Thirsty degree scores compared with KT/V value, serum sodium concentration、mean arterial pressure、the patient’s age and the average age of dialysis, which were no significant correlation (P>0.05).(6)In addition to the above indicators, we also found that the patient’s insomnia、much dream symptoms、bloating、constipation and tinnitus,which were relieved in addition to the thirsty symptoms improved obviously after the treatment of the above methods during the process of experiment.Conclusion(1) Ear point-pressing joint chewed medlar can improve the thirst of maintenance hemodialysis patients.(2) Ear point-pressing joint chewed medlar can control weight gain between dialysis period.(3) Ear point-pressing joint chewed medlar improve patients’thirsty and control weight gain between dialysis period which were increased with the increase of course, the longer the course of treatment the better the results is.(4) Thirsty result in dialysis patients drinking too much and too fast, lead to dialysis interphase weight increased too much, which is the main reason for the increased mortality and heart, cerebrovascular complications.By means of ear point-pressing joint chewed medlar improving the patients’ symptoms of thirsty, controlling weight gain between dialysis period which reducing the heart, cerebrovascular complications, improving the patients quality of life. |
