Western medicine treatment of m alignant ascites mainly includes diuresis and albumin supplementation, abdom inal puncture drainage, systemic chemotherapy, intraperitoneal perfusion che motherapy, and so on. Various treatment has certain effectiveness, but has i ts limitations. such as diuresis and albumin supplementation are palliative and easy to cause loss of nutrients, abdominal puncture drainage is paid of f, but the long-term effect is poor. systemic chemotherapy and intraperitone al perfusion chemotherapy are main western medicine treatment methods, but m ost malignant ascites are appeared in the late stages of tumor progression, general condition is poor, KPS score is low, malignant ascites patients may receive much cycles of chemotherapy, which leads to the sensitivity of the chemotherapeutics reduced, So the patient’s physical and psychological are difficult to receive systemic chemotherapy and intraperitoneal chemotherap y. Therefore, it is urgent to find a good method of treatment that is curat ive effect, low side effect, wide indications, middle-late tumor patients ca n accept. External therapy of traditional Chinese Medicine can make drug ac t directly on the lesion, and can increase local curative effect and allevia te the adverse reaction of the body. Traditional Chinese medicine abdominal cavity perfusion therapy has many advantages,such as good curative effect, less side effects, middle-late tumor patients can accept, which is more an d more valued. Our department using HuaChansu injection intraperitoneal per fusion treatment of malignant ascites has achieved good clinical effect, an d our department has made some preliminary clinical observation, which foun d it certain inhibitory effect on the growth of malignant ascites.[Research purpose]Through clinical observation, Studying of Huachansu injection intraperit oneal perfusion treatment of malignant ascites, Observing the changes of the quantity and quality of malignant ascites,Observing the changes of symptom integral and Karnofsky after Huachansu injection intraperitoneal perfusion treatment. then to observe the curative effect difference between digestive system malignant ascites and non digestive system malignant ascites, to obs erve the curative effect difference between local dialectical damp-heat tox in syndrome and cold-damp toxin syndrome. In order to obtain a more specifi c, more targeted results, to guide clinical treatment.[Research methods]Adopting the method of retrospective study, To Observe102patients are chosen who meet the inclusion criteria during January2010to December2013in Oncology of Dongfang Hospital of Beijing University of Chinese Medicin e, Observing the changes of the quantity and quality of malignant ascites, a fter completing1course (2weeks) of Huachansu injection intraperitoneal p erfusion treatment; Observing the changes of symptom integral and Karnofsky after completing1course (2weeks) of Huachansu injection intraperitoneal perfusion treatment. Respectively to observe the curative effect difference between digestive system malignant ascites and non digestive system malign ant ascites, the curative effect difference between local dialectical damp-h eat toxin syndrome and cold-damp toxin syndrome. Observation of quantity of malignant ascites extends to1month after treatment. Using SPSS20statist ical software, to establish a database and make statistical analysis.[Research results]①The curative effect of quantity of malignant ascites:Complete remiss ion (CR)5cases, partial remission (PR) in27cases, stable disease (SD)35cases, progressive disease (PD)35cases, efficiency (CR+PR+SD) is65.7%. calculated the digestive system malignant ascites efficiency is75.4%, the non digestive system malignant ascites efficiency is51.2%, Compared the tw o groups (P<0.05), the difference has statistical significance, show that the curative effect of Huachansu injection intraperitoneal perfusion treatm ent in digestive system malignant ascites is better than that of non digest ive system malignant ascites; calculated the damp-heat toxin syndrome malign ant ascites efficiency is70.1%, the cold-damp toxin syndrome malignant asci tes efficiency is40.0%, Compared the two groups (P<0.05), the difference has statistical significance, show that the curative effect of Huachansu inj ection intraperitoneal perfusion treatment in damp-heat toxin syndrome mali gnant ascites is better than that of cold-damp toxin syndrome malignant asc ites.②The curative effect of quality of malignant ascites:after Huachansu i njection intraperitoneal perfusion treatment,102patients with malignant a scites the level of red blood cells (RBC), lactate dehydrogenase (LDH), tum or markers (TM) all decrease significantly compared with those before treat ment. Compared after treatment with before treatment, the level of red bloo d cells decreases≥25%accounted for61.8%, the level of LDH decreases≥25%accounted for63.7%, the level of TM decreases≥25%accounted for578%. Layered statistics show, In the aspect of quality (RBC、LDH、TM) improvem ent, Compared digestive system malignant ascites with non digestive system malignant ascites, all P<0.05, there are statistical significance. In the a spect of quality(RBC、LDH、TM) improvement, Compared the damp-heat toxin sy ndrome malignant ascites with the cold-damp toxin syndrome malignant ascite s, all P<0.05, there are statistical significance. It show that Huachansu i njection intraperitoneal perfusion treatment is more effective on digestive system malignant ascites and the damp-heat toxin syndrome malignant ascite③symptom integral and Karnofsky:After Huachansu injection intraperit oneal perfusion treatment, the symptom integral decreases from (11.39±3.57score) to (8.91±2.76score), Compared after treatment with before treatm ent, P<0.05, the difference has statistical significance. After Huachansu injection intraperitoneal perfusion treatment, the Karnofsky increases from (51.27±0.99score) to (59.02±1.18score), Compared after treatment with b efore treatment, P<0.05, the difference has statistical significance. It s how that Huachansu injection intraperitoneal perfusion treatment can signif icantly reduce those symptom which caused by malignant ascites, such as abdo minal distension, abdominal pain, nausea and vomiting, anorexia, oliguria, breathing difficulties and other symptoms, and can improve the physical cond ition of patients effectively.④Safety evaluation and side effects:After Huachansu injection intrape ritoneal perfusion treatment,102patients all not happen liver and kidney function, bone marrow suppression, cardiac toxicity and other serious adver se reactions. Common side effects were abdominal pain, diarrhea, fever, the rates were13.73%,4.90%,21.57%.[research conclusions]①The study show that Huachansu injection intraperitoneal perfusion tr eatment malignant ascites is an effective treatment method, which deserves further research and application②The study show that Huachansu injection intraperitoneal perfusion tr eatment is more effective on digestive system malignant ascites and the dam p-heat toxin syndrome malignant ascites.
