Chinese Medicine Combined With Western Medicine Clinical Efficacy Of Dialectical Treatment Of Diabetic Nephropathy

ObjectivesThrough the observation of intergrated traditional Chinese medicine and western medicine treatment intervention in diabetic nephropathy,summarize the advantages about traditional Chinese medicine(TCM) combined with western medicine in the treatment of diabetic nephropathy(DN),to lay the foundation for the further study and clinical experiences of Chinese medicine treatment of diabetic nephropathy.MethodsA prospective study of randomization method:collect patients with DN III, IV stages from Wangjing Hospital,Jiangtai community service centers,Fatou Community service in-patient or out-patient between Jan2013to June2013.And divide the patients int two groups,30patients for the treatment group to accept the treatment of traditional Chinese medicine combined with western medicine,the other30patients as the control group,treated with western medicine.The treatment course is3months.Compare the differences between the two groups in clinical indicators,observe the efficacy of clinical symptoms, proteinuria,renal function,the compliance rate of blood glucose,lipids,blood pressure.the rate of the endpoint event before and after the treatment.And observe the curative effect of integrated traditional Chinese medicine and western medicine treatment intervention in diabetic nephropathy.Results(1)Proteinuria curative effect:In treatment group markedly effective rate of proteinuria decreasing was26.7%,efficiency was43.3%and Total efficiency was70%. In control group,markedly effective rate of proteinuria decreasing was10.3%, efficiency was24.1%and Total efficiency was34.5%. Total efficiency by x2test, P<0.01,the difference was markedly statistical significant.(2)Renal functionr:In treatment group markedly effective rate of renal function was16.7%,efficiency was36.7%and Total efficiency was53.4%. In control group, markedly effective rate of renal function was6.9%, efficiency was17.2%and Total efficiency was24.1%. Total efficiency by x2test, P<0.05,the difference was statistically significant.(3)The compliance rate of blood glucose,lipids,blood pressure:The compliance rate of glycosylated hemoglobin of treatment group was83.3%, while the control group was24.1%. The compliance rate by x2test,P<0.01, the difference was markedly statistical significant between two groups. The compliance rate of blood pressure in the treatment group was63.3%, while the control group was27.6%. The compliance rate by x2test,P<0.01, the difference was markedly statistical significant between two groups.The compliance rate of CHO of treatment group was60%,while31%of the control group. The compliance rate by x2test,P<0.05, the difference was statistically significant between two groups.The compliance rate of TG of treatment group was56.7%,while27.6%of the control group. The compliance rate by x2test,P<0.05, the difference was statistically significant between two groups.The compliance rate of HDL of treatment group was53.3%,while31%of the control group. The co-mpliance rate by x2test,P>0.05, the difference was no statistically significant.(4)Clinical main symptoms:In treatment group markedly effective rate of clinical main symptoms was40%,efficiency was40%and Total efficiency was80.0%. In control group,markedly effective rate of clinical main symptoms was6.9%,efficiency was34.8%and Total efficiency was41.4%. Total efficiency by x2test, P<0.01,the difference was markedly statistical significant.(5)The rate of the endpoint event:Ⅲ stage DN of proteinuria endpoint event rate was6.7%in treatment group and11.8%in control group. IV stage DN of proteinuria endpoint event rate was13.3%and the control group was33.3%. the overall incidence rate of proteinuria endpoint event rate was10%in treatment group,20.7%in control group. The total incidence of periods DN renal function endpoint event was6.7%in treatment group was20.7%in control group. From above data between the two groups, there was no statistically significant difference P>0.05. There were no deaths in two groups.ConclusionsTCM combined with western medicine treatment can effectively decrease proteinuria, protect renal function,improve the rate of compliance about blood glucose lipids and blood pressure,improve clinical symptoms, improve the survival quality of patients.
