Background and ObjectiveEndometriosis is known as benign lesion, while it is also characterized by its benign and malignant biological behavior traits. The mechanism of endometriosis associated ovarian cancers remains unknown since it was firstly opposed by Sampson in1925. One of the findings from investigations nowadays is that HNF-1β, which is associated with the developing of ovarian clear cell cancer, is also expressed in endometriosis. The relationship of HNF-1β between ovarian cancers and endometriosis is still unclarified. Another research assumed that the malignant change of endometriosis is associated with the bleeding of the lesion which induces the increase of oxidant stress in the lesion and the probable factor increasing the oxidant stress is iron. The objective of this research is (1)to confirm the effect of HNF-1β in endometriosis, ovarian cancers and EAOC;(2)to confirm the expressing difference of HNF-1β in different types of ovarian cancers or tumors;(3)to confirm the relationship between HNF-1β and Fe in the development from endometriosis to ovarian cancers.MethodsCases admitted to PUMCH for surgery with diagnosis both of endometriosis and ovarian cancers/tumors were collected. The inclusion criteria was (1) the operation tissue contains both of ovarian cancers/tumors and endometriosis (2) the history of endometriosis is longer than the history of ovarian cancers/tumors. The exclude criteria was that the ovarian cancers were transferred from other organs. Pathological tissue of18patients was collected. Immunohistochemical stain of HNF-1β and Perls stain of hemosiderin were performed. Image digital collection system and Image Pro Plus software were used to analyze the stain results(mean optical density, positive nuclear percentage and positive area percentage) of the slides. SPSS17.0was used to analyze the data. ResultsThere is no statistically significant difference between cases with and without endometriosis in the mean optical density of ovarian cancers and ovarian borderline tumors. The expressing level of HNF-1β was statically higher in the nuclear of clear cell cancer cells and borderline tumor cells. In adenomyosis, the MOD is decreased gradually from endometrial cells to smooth muscle cells around, however the differences are not significant. As to ovarian clear cell cancers, the MOD of HNF-1β in positive cells is significantly higher than the MOD in endometriosis and normal endometrium. Besides, the positive nuclear percentage is significantly higher than serous papillary cancers, transformation cell cancers and ovarian endometriosis.There are no significant differences of iron disposition between cancers, ovarian endometriosis cyst and uterine endometriosis,while there is positive correlate relation ship between endometriosis cyst and endometriosis lesion in the uterus for the same case.In cancer cells, the relationship between the MOD of HNF-1β and Fe is negative correlated, while in endometriosis lesions there are no significant differences between the two factors.ConclusionHNF-1β contained in endometriosis give the clear cell in ovaries the potential of malignantly changing to ovarian clear cell cancers. Fe might have effect in the course of endometriosis malignant change, however there is no clear correlate between Fe and HNF-1β.
