The Aesthetic And Functional Correction For The Deviated Nose Deformity

Objective To explore the characteristic of traumatic and Unilateral cleft lip deviatednose deformity with nasal airway obstruction and develop an individualized syntheticsurgical method to rebuild nasal aesthetic form and nasal airway function.Methods18cases of deviated noses with deviated septums,coming from Department ofPlastic Surgery in Anhui Provincial Hospitial,during January2011to December2013,15cases of men and women in3cases,aged16-35,average age25.8, disease period6monthes to32years.12cases are due to trauma and6cases are Unilateral cleft lip. Thecorrective techniques for deviated septum include Septoplasty and spreadergraft.Classifying the bony pyramid of traumatic deviated nose according to Burm’sclassification, and the individual osteotomy was adopted in unilateral bony mobilisation.The special type osteotomy was used in unilateral cleft lip patients. With combinationof cartilage grafts for nasal dorsum and sigmasubunits, resection of the inferiorhypertrophy turbinate,comprehensive personalized treatment was used for correcting thedeviated nose deformity.Following up3-12monthes,evaluated the nasal shape and nasalcongestion according to VAS subjective assessment,meanwhile objective nasalresistance determination and the vertical distance of the lateralst point to the face centerline was also messured.Results The deviated bony pyramid and nasal septum were returned to midline in allcases.The aesthetic line of the nasal dorsum reappeared and the nasal airway wassignificantly improved, both patients and doctors felt satisfatory,with no complicationslike septum perforation,hematoma,infection and others following up3-12monthes,except mild deformity recurrence occurred in3patients.Conclusion At present, there is no standard method can deal with all types of aesthetics and functional promblems of the deviated nasal deformity.Because of the complicated anatomical structure,dysfunction and various deformity,preoperative strict physical examination, systematic nasal analysis,identification ofdeformity, affluent knowledge about anatomical relationships were all needed toestablished common goals with patients. Comprehensive technologies includingseptoplasty,spreader graft, the individual osteotomy, cartilage grafts for nasal dorsumand sigmasubunits could be used to restore the important frame support, reconstruct thenasal aesthetic form and improve the nasal airway. Satisfactory results have beenachieved.
