Objective:To determine great clinical significance of MMP-9and D-dimer as biological markers to predict the condition changes and prognosis of acute cerebral infarction and different degrees of carotid stenosis by analyzing their relationships among MMP-9and D-dimer to traditional risk factors in patients with acute cerebral infarction, and by detecting the levels of matrix metalloproteinase-9(MMP-9) and D-dimer in serum of patients with acute cerebral infarction and patients with different degrees of carotid stenosis.Methods:87patients with ACI were enrolled in department of neurology of Shandong Provincial Qianfoshan Hospital from January to October in2013.According to clinical syndrome, MRI and carotid arterycolor Doppler ultrasongraphy all patients were divided into3groups.15patients in severe carotid stenosis group,56with slight-moderate carotid stenosis and16without carotid stenosis.121healthy person were in normal control group. Peripheral blood MMP-9levels were detected by ELIS A method and D-dimer levels were detected by coagulation method, at the same time, the levels of peripheral blood WBC, HB, PLT, MPV, PDW, PCT, PT, APTT, CHOL, TG, HDL, LDL, LPa, CRP and GLU were measured. All data were analysed by SAS17.0, and a P<0.05value was considered significant.Results:The serum concentration of MMP-9and D-dimer and traditional risk factors(RFs) including blood pressure, PLT, MPV, HDL,TG and CRP in ACI group was higher than that in normal control group (P<0.05). While the other RFs (WBC, HB、PT、APTC、CHOL、LDL、LP (a)、GLU、PDW、PCT) showed no difference compared with normal control group (P>0.05). Moreover, among ACI cases, MMP-9and D-dimer were significantly higher in the two stenosis groups than in the group without stenosis (P<0.05). Both MMP-9and D-dimer level were positively related with the severity of the carotid stenosis by spearmen correlation analysis (r=0.408, P<0.01). And otherwise, MMP-9was positively related with D-dimer and LP(a)(r=0.2100, P<0.05; r=0.2290, P<0.05) negatively related to HDL(r=-0.215, P<0.05), but showed no relationship with RFs including WBC^HB、PT、APTT、CHOL and LDL (P>0.05).Conclusions:Serum MMP-9and D-dimer level have close relationship with onset and development of ACI and the severity of the carotid stenosis. MMP-9and D-dimer joint detection can be used as sensitivity markers for acute cerebral infarction early diagnose and prognosis, they can provide important clinical laboratory evidence for cerebral infarction diagnose, treatment effect observation and prognosis.
