Objective To observe the structure changes of ocular anterior segmentafter trabeculectomy by using ultrasound biomicroscopy(UBM) in acuteangle—closure glaucoma and chronic angle—closure glaucoma.Methods18patients(26eyes)of chronic angle-closure glaucoma and24patients(31eyes) of acute angle—closure glaucoma trabeculectomy wereunderwent.The structure of ocular anterior segment were measured by UBMpre-operation and post-operation separately,and compare the consistency of thechamber angles examined by using goniscope and UBM.The parametersincluded anterior chamber distance(ACD),trabecular iris angle(TIA), angleopening distance(AOD500),trabecular ciliary processes distance(TCPD) andiris distance(ID1).Results1. Ciliary detachment was found in1patient(1eye)postoperatively.All patients’s intraocular pressure(IOP) were controlled between6to18.5mmHg after surgery.The mean IOP was13.2±4.5mmHg.2.In the apart of chronic-angle closure glaucoma and the apart of acuteangle-closure glaucoma patients patients,the chamber angle was nealy the same. 3. Using UBM determines the filtrable passage after trabeculectomy1monthlater.Between conjunctiva flap and sclera flap,there is a echolessarea can befound.No ciliary body detachment was found. Aqueous homor was foundflowing freely.4.In acute angle—closure glaucoma part and in chronicangle-closure glaucoma patients,there were no significant differences betweenthe preoperative and postoperative AOD,TIA,ACD,TCPD, ID1(P>0.05).Conclusion1.UBM is unique to observe the filtrable passage in vivo andestimate whether it is obstructed or not.It posses higher value in evaluating thefunction of filter bleb.2.Intraocular hypertension is relieved aftertrabeculectomy.But the risk factors of lens and ciliary body Can’t berelieved.3.UBM is an exactly tool to measure the changes of the structure ofocular anterior segment after trabeculectomy.It posses higher diagnostic valuein discussing mechanism of glaucoma,reducing postoperative complication andevaluating therapeutic effect....
