Objective: To evaluate the changes in the anterior segmentmorphology by using ultrasound biomicroscopy (UBM) after YAG laserperipheral iridotomy (LPI) in eyes with primary angle closure (PAC).Methods:From March2012to March2013,28patients (48eyes) whodiagnosed with PAC and underwent LPI in our hospital, are regarded as theresearch object. By using the UBM, the following issues are observed onthe superior temporal, above, below, temporal, and nasal side of the eyeballat1day,1week,1month and3months with the preoperative andpostoperative of LPI: angle opening distance (AOD500), trabecular irisangle (TIA), trabecular ciliary process distance (TCPD), iris ciliary processdistance (ICPD), iris lens angle (ILA), iris lens contact distance (ILCD), iristhickness1(ITl), iris thickness2(IT2), iris thickness3(IT3), and the anteriorchamber depth (ACD) were included.Results:1. AOD500, TIA were increased significantly (P<0.05), compared to before LPI; and the postoperative1week was more than the rest of the timeafter surgery (P>0.05).2. TCPD was increased significantly, compared to before LPI (P <0.05), excepted the increase at the time of postoperative1day andpostoperative3months on below side were no statistically significant (P>0.05); and postoperative1week were more than the others after surgery(P>0.05).3. There were no significant differences when a comparision of theICPD (P>0.05).4. ILA were decreased in the postoperation compared to thepreoperation (P <0.05); And ILA were increased gradually from1day to3months after the operation (P>0.05).5. ILCD were increased after LPI (P <0.05); In contrast with the ILA,ILCD were decreased gradually from1day to3months after the operation(P>0.05); Specifically, compared to postoperative1day on the temporal,nasal and below side, postoperative1week on the nasal and lower side,postoperative1month on the nasal side, ILCD at postoperative3monthswas decreased (P <0.05).6. IT1, IT2, IT3were decreased in the postoperation compared to thepreoperation (P <0.05), excepted the mean IT1, IT2at the time ofpostoperative3months. The mean IT1, IT2and IT3were decreasedgradually from1day to3months after the operation, which the mean IT1 at the time of postoperative3months was increased compared topostoperative1week, and the mean IT2, IT3at the time of postoperative3months was increased compared to postoperative1day(P <0.05).7. ACD of postoperation was increased compared to before LPI (P>0.05); and the difference of ACD were not significant between eachpostoperative times (P>0.05).8. The mean ILA and mean ILCD correlation shown that the two werehighly negative correlation on every time points by Pearson correlationcoefficient analysis.Conclusion: The peripheral anterior chamber of PAC patients after theLPI were obviously deepened, the iris bombe and pupillary block wererelieved after LPI, but the anterior segment morphology in the3months ofpostoperation were not stable, individual reference of which had somedegree of retrograde. Therefore, long-term follow-up is necessary after LPI.
